摘要
目的探讨早期综合康复方案对脑卒中后急性肩手综合征(SHSAS)患者的临床效果。方法方便选取2013年10月—2015年10月临淄区妇幼保健院内一科收治的134例SHSAS患者,随机分为观察组和对照组,每组67例。对照组给予脑卒中后常规康复锻炼,观察组采取早期综合康复方案,两组均治疗4周。采用Fugl-Meyer(FMA)评估两组患者治疗前后上肢运动功能,采用视觉模拟评分法(VAS)评估疼痛程度。随访3个月,采用WHOQOL-BRIEF量表评价两组患者治疗前后生活质量。结果观察组FMA评分(20.65±2.32)分,明显高于对照组(P<0.01),VAS评分(3.67±0.48)分,明显低于对照组(P<0.01);观察组生理领域、心理领域、社会领域、环境领域得分分别为(67.72±6.46)分、(68.35±6.86)分、(67.36±6.47)分、(65.45±5.98)分,均显著高于对照组(P<0.05)。结论观察早期综合康复方案有助于提高脑卒中后肩—手综合症患者的上肢功能,减轻疼痛程度,改善患者的生活质量。
Objective To discuss the clinical effect of early comprehensive rehabilitation plan on the acute shoulder-hand syndrome after stroke. Methods 134 cases of SHSAS patients admitted and treated in our hospital from October 2013 to October 2015 were conveniently selected and randomly divided into two groups with 67 cases in each, the control group and the observation group were respectively treated with routine rehabilitation exercise after stroke and early comprehensive rehabilitation plan for four weeks, and the upper limb motor function before and after treatment of the two groups were evaluated by the Fugl-Meyer（FMA）, and the pain degree was evaluated by the VAS, and the quality of life before and after treatment of patients was evaluated by the Fugl-Meyer（FMA） scale. Results The FMA score in the observation group was（20.65±2.32）points, which was obviously higher than that in the control group（P〈0.0）1, and the VAS score in the observation group was（3.67±0.48）points, which was obviously lower than that in the control group（P〈0.01）, and the physical field,mental field, social field, and environmental field score were respectively（67.72±6.46）points,（68.35±6.86）points,（67.36±6.47）points,（65.45 ±5.98）points, which were obviously higher than those in the control group（P〈0.05.） Conclusion The early comprehensive plan contributes to improving the upper limb function of patients with acute shoulder-hand syndrome after stroke, relieve the pain degree and improve the quality of life.
