摘要
目的研究肾上腺素用量与心肺复苏(CPR)的相关性。方法回顾自2012年1月-2014年12月在院内或院外发生心搏骤停(CA)后,经我院急救中心抢救的患者393例。将入选病例按CPR时间分为常规组190例和超长组203例。分别分析各组自主循环回复率(ROSC)、出院存活率、30天良好神经功能恢复率与肾上腺素用量之间的相关性。结果常规CPR组ROSC、出院存活率、30 d良好神经功能恢复率与肾上腺素剂量的Pearson相关系数分别为:-0.633,-0.394,-0.382。超长CPR组ROSC、出院存活率、30 d良好神经功能恢复率与肾上腺素剂量的Pearson相关系数分别为:-0.244,-0.202,-0.120。结论增加肾上腺素剂量,并不会改善CPR患者的短期存活率或远期存活质量。甚至对于常规CPR患者,肾上腺素剂量和ROSC率呈中度的负相关。
Objective To explore the relationship between the dosage of adrenaline and prognosis from eardiopulmonary resuscitation. Methods 393 patients with cardiac arrest （CA） in our emergency center from January 2012 to December 2014 were recruited. According to cardiopulmonary resuscitation （CPR） time, patients were divided into conventional group and long time group. The Pearson correlation coefficients were estimated between prognosis and adrenaline used dose. Results In conventional CPR group, the correlation coefficients between restoration of spontaneous circulation （ROSC） rate, the survival to hospital discharge, 30 days good neurological recovery and the dosage of adrenaline were -0.633, -0.394 and -0.382 respectively. In long time CPR group, the correlation coefficients between ROSC rate, the survival to hospital discharge, 30 days good neurological recovery and the dosage of adrenaline were -0.244, -0.202 and -0.120 respectively. Conclusion Increasing the dosage of adrenaline in cardiac arrest failed to increase the rates of achieving ROSC, the survival to hospital discharge or 30 d good neurological recovery. The relationship between ROSC rate and the dosage of adrenaline in patients who underwent conventional CPR is a moderate negative correlation.
