摘要
目的:探讨床边颅脑彩超在开颅去骨瓣减压术后成人患者中的应用价值。方法:选取2014年6月-2016年12月本院重症医学科收治的脑外伤及高血压脑出血行去骨瓣减压术的成年患者32例,经骨窗超声检查再发血肿、脑梗死、脑挫伤灶等,并与颅脑CT进行对比研究。结果:床边颅脑彩超在术后发现病灶18例,CT发现病灶21例,符合率高达85.71%。结论:经去骨瓣减压窗,床边超声可以准确探测术后颅内再次出血、脑梗死及脑挫伤灶,所测量脑内血肿量、脑挫裂伤灶大小、脑梗死面积与CT测量值相关性显著。
Objective: To explore the application value of bedside ultrasound in skull-flap removal and eraniotomy in adult patients.Method: From June 2014 to December 2016, 32 patients admitted to the department of critical care medicine of Houjie Hospital of Guangdong Medical University who underwent deeompressive craniectomy were studied, through the bone window ultrasound examination, again the haematoma, the cerebral infarction, the brain contusion and so on, and compared with brain CT.Result: 18 cases of lesions were found by bedside cranial color Doppler ultrasound, 21 cases were found by CT, and the coincidence rate was 85.71%. Conclusion: Through the decompressive eraniectomy window, bedside cranial color Doppler ultrasound can accurately detect postoperative intracranial hemorrhage, cerebral infarction and contusion of brain, the amount of intracerebral hematoma, the size of brain contusion and laceration, the area of cerebral infarction and the value of CT measurement are significantly correlated.
