摘要
目的:探讨血清EB病毒抗衣壳抗体(VCA)IgM、IgA、IgG在小儿传染性单核细胞增多症(IM)诊断中的临床应用价值。方法:采用ELISA方法同时检测50例IM患儿血清标本及同期50例健康儿童血清标本中的VCA-IgM、VCA-IgA、VCA-IgG抗体含量,并将各组间结果进行分析,比较差异。结果:VCA-IgM检测:IM敏感度(92%)、特异度(96%)、阳性预测值(96%)、阴性预测值(92%)。患儿92%的阳性率明显高于体检对照组4%(P<0.05)。VCA-IgA检测:IM患儿阳性率为6%,体检对照组为2%,两组区别无统计学意义(P>0.05)。VCA-IgG检测:IM患儿敏感度为58%、特异度为74%、阳性预测值为72%、阴性预测值为65%。其患儿阳性率明显高于对照组(P<0.05)。结论:比较EBV-VCA-IgM、IgA、IgG三种抗体在IM疾病中的诊断,其中以IgM其敏感度、特异度、预测值最理想;I-gA在患儿及对照组中均检出率低;IgG虽灵敏度、特异性、阳性预测值均满意,但65%的阴性预测易造成假阳性。
Objective To investigate the application of three Epstein-Barrvirus(EBV) antibodies(VCA-IgM、 VCA-IgA、VCA-IgG) in diagnosing IM children.Methods The serum of 50 well children were detected using enzyme-linked immunosorbent assay(ELISA).Results 46 cases had positive VCA-IgM(92%).The mean IM copies of diagnosed group were significantly higher than that of negative control group(P<0.05).3 cases had positive VCA-IgA(6%).29 cases had positive VCA-IgG(58%),but the sensitivity and specificity is very low.Conclusion Positive rate of IM determined by the assay of VCA-IgM is higher than that by VCA-IgA and VCA-IgG.Analyzing VCA-IgM could be helpful diagnosis of IM especially for those patients with suspected EBV infection.
