摘要
目的观察不同早期康复护理干预方式对出血性脑卒中患者术后康复的影响。方法选择2014年6月~2016年5月在青岛市市立医院就诊的出血性脑卒中手术患者194例,采用随机数字表法将患者分为对照组和研究组,每组各97例。对照组在术后2~7 d患者生命体征稳定后,给予早期康复护理干预;研究组在术后2~7 d患者生命体征稳定后,给予反馈式早期康复护理干预,两组护理干预计划均持续至出院后6个月。比较两组康复煅炼的时间和次数依从性、Barthel指数及生活质量评分。结果研究组住院期间和出院后时间依从率、次数依从率均显著高于对照组(P<0.05)。两组出院时、出院1个月时、出院6个月时Barthel指数均显著高于护理前(P<0.05),且研究组高于同期对照组(P<0.05)。两组护理后躯体角色、躯体疼痛、躯体功能、总体健康、活力、社会功能、情绪状态、精神健康均高于护理前(P<0.05),且研究组高于同期对照组(P<0.05)。结论早期反馈式康复护理干预为出血性脑卒中术后早期护理康复干预提供了新模式,值得临床推广应用。
Objective To study the effect of different early rehabilitation nursing intervention on postoperative rehabilitation of patients with hemorrhagic stroke. Methods 194 patients with hemorrhagic stroke in Qingdao Municipal Hospital from June 2014 to May 2016 were selected and divided into control group and study group, with 97 cases in each group. The control group was received early rehabilitation nursing intervention at 2-7 d after operation and the patient＇s vital signs were stable, while the study group was received feedback early rehabilitation nursing intervention at 2-7 d after operation and the patient＇s vital signs were stable. The nursing intervention program of two groups continued until 6 months discharge. The time compliance and frequency compliance, Barthel index and quality of life score before and after nursing of two groups were compared. Results The time compliance and frequency compliance during hospitalization and after discharge of study group were significantly higher than those of control group, with statistically significant difference（P 0.05）. The Barthel index of two groups at discharge time, 1 months, 6 months after discharge were significiantly higher than those before nursing（P 0.05）, and Barthel index of study groups at discharge time, 1 months, 6months after discharge were significiantly higher than those of control group（P 0.05）. The body role, body pain, body function, general health, vitality, social function, emotional state, mental health of two groups after nursing were significantly higher than those before nursing（P 0.05）, and those of study group were significiantly higher than those of control group（P 0.05）. Conclusion The early feedback rehabilitation nursing intervention provides a new model for early postoperative nursing intervention in patients with hemorrhagic stroke, which is worth to be spread.
