肝纤维化分级的MRI扩散加权成像与超声剪切波弹性成像对比分析 被引量:1

摘要 目的 探讨MRI扩散加权成像与超声剪切波弹性成像(SWE)对肝纤维化的诊断价值,并比较分析两者的临床意义.方法 65例我院收治的肝纤维化患者同时行MRI扩散加权成像和超声剪切波弹性成像(SWE),测定ADC值和杨氏弹性模量值(Emean),行单因素方差分析和LSD组间比较,采用Spearman等级相关分析肝纤维化程度与ADC值和Emean值的相关性,采用ROC曲线分析ADC值和Emean值的效能.结果 随着肝纤维化程度加重,ADC值依次减低(P〈0.01)、Emean值依次增高(P〈0.01).ADC值与肝纤维化程度呈负相关(r=-0.821),Emean值与肝纤维化程度呈正相关(r=0.919).经ROC曲线分析显示,ADC值和Emean值诊断不同等级肝纤维化均具有较高的效能,但Emean值更具优势.结论 在肝纤维化病理分级及相关性分析中,Emean值和ADC值对诊断不同等级肝纤维化有较高的价值,超声剪切波弹性成像更具优势. Objective To investigate the diagnosis value of DWI and SWE in liver fibrosis, compare and analyze their clinical significance. Methods DWI and SWE were carried out in 65 cases of patients with liver fibrosis, the ADC and Emean values were measured, single factor variance analysis and LSD were compared between the two groups. The Spearman rank correlation analysis for liver fibrosis degree and the correlation of ADC and Emean value were carried out. The ROC curve analysis were carried out to analyze the efficiency of ADC and Emean values. Results With the liver fibrosis degree aggravating, ADC values reduced (P〈0.01) and the Emean values increased (P〈0.01). ADC value was negatively related to the degree of liver fibrosis (r=-0.821), Emean value was positively associated with the degree of liver fibrosis (r=0.919). The ROC curve analysis showed that ADC values and Emean value has higher efficiency in the diagnosis of different degree of liver fibrosis, but Emean value has more advantages. Conclusion In liver fibrosis pathological classification and correlation analysis, Emean value and ADC value has high value to the diagnosis of different degree of liver fibrosis, and SWE has more advantages.
