呼吸管理团队在COPD伴呼吸衰竭序贯机械通气患者中的应用 被引量:17

摘要 目的:探讨呼吸管理团队在慢性阻塞性肺病(COPD)伴呼吸衰竭序贯机械通气患者中的应用效果。方法:选取2014年1月~2016年12月本院收治的COPD伴呼吸衰竭序贯机械通气患者100例,将患者随机等分为观察组及对照组,对照组行常规性护理管理,观察组成立呼吸管理团队实施综合康复护理,比较两组患者预后情况及并发症发生情况。结果:观察组患者干预后急性病理生理和慢性健康评分(APACHEⅡ)低于对照组(P<0.05),呼吸机使用时间、住ICU时间、平均住院时间均短于对照组(P<0.05),患者呼吸机相关并发症、不良事件、护理投诉发生率均低于对照组(P<0.05),治疗满意率高于对照组(P<0.05)。结论:呼吸管理团队能有效降低COPD伴呼吸衰竭序贯机械通气患者机械通气并发症发生率,缩短住院时间,能有效促进患者康复。 ObjectiveTo discuss the application effect of respiratory management team in chronic obstructive pulmonary disease (COPD) accompanying re-spiratory failure sequential mechanical ventilation. Methods: Selected 100 patients with COPD accompanying respiratory failure sequential mechanical venti-lation admitted in our hospital from January 2014 to December 2016 , and they were equally divided into observation group and control group, where patients in the control group received conventional nursing management, and for the patients in the observation group, a respiratory management team was estab-lished for comprehensive rehabilitation nursing, and patients in the two groups were compared in prognosis and complications. Results : After intervention, the acute pathophysiology and chronic health evaluation (APACHE II ) of patients in the observation group were lower than those of patients in the control group (P 〈 0. 05) , the time to use respiratory machine, length of stay in ICU and average length of stay of patients in the observation group were shorter than those of patients in the control group (P 〈 0. 05) , the respiratory machine related complications, adverse events and nursing complaint incidence rate of pa-tients in the observation group were lower than those of patients in the control group(P 〈0. 05) , and the treatment satisfaction rate of patients in the obser-vation group was higher than that of patients in the control group( P 〈0.05 ). Conclusion : Respiratory management team could effectively reduce the inci-dence rate of complications of patients with COPD accompanying respiratory failure sequential mechanical ventilation, shorten the length of stay and could effectively promote the patients ’ rehabilitation.
