摘要
目的 :基于前期研制的高血压合并颈动脉粥样硬化(CAS)证素诊断量表探索高血压合并CAS人群中证候分布规律,并以此指导临床治疗策略的制订。方法:采集高血压合并CAS患者临床资料,通过量表评估其证素成立情况,统计分析证候分布规律。结果:高血压合并CAS多见于老年人,男性发病年龄早于女性,男女证素成立情况不同;证素成立最多的为痰证,病性证素成立多于病位证素;吸烟对火热、痰、血瘀证素的成立有影响(P<0.05);痰、血瘀、火热、肝证素的成立对斑块面积有影响(P<0.05);证素组合情况中以三证素最多见,其次为四证素、二证素、五证素、一证素的组合形式。结论 :痰+火热、血瘀+火热、火热+阴虚、痰+血瘀、血瘀+阴虚是证素组合的5种基本结构,痰热、瘀热、火热伤阴、痰瘀互结、阴虚血瘀是高血压合并CAS患者临床常见证候。
Objective：Explore the syndrome distribution based on the pre-developed hypertension with carotid atherosclerosis （CAS） syndrome elements diagnosis scale and to guide the development of clinical treatment strategies,in order to guide the development of clinical treatment strategies. Methods：Collect clinical information of the patients,assess syndrome elements founded circumstances through the scale and explore the syndrome distribution. Results：Hypertension with CAS often occured in the elderly,the age of onset of male earlier than females. Founded situation of syndrome elements was different between the sexes. The most common syndrome element was sputum. The syndrome elements of disease location was more than the elements of disease resistance. Greater impact of smoking on fiery,phlegm,blood stasis （P〈0.05）. Phlegm,blood stasis,fiery,liver had more affection on plaque area（P〈0.05）. In the founded syndrome cases,three kinds of syndrome elements combination were the most common situation,followed by four syndrome elements,two syndrome elements,five syndrome elements combination. Con-clusion：Five basic structure of the syndrome element combination include phlegm＋fiery,blood stasis＋fiery,fiery＋deficiency, phlegm＋blood stasis,blood stasis＋deficiency. These five are also common basic syndromes in hypertension with CAS.
