摘要
基于公开发表文献的相关研究发现,美国医疗信息交换(HIE)的利益相关者(包括医疗机构、医护人员、消费者、政策制定者等)对于HIE在提高医疗保健质量和减少成本方面的价值具有高度一致的认可,但是对于接受和使用HIE有不同的考量。医疗机构和医护人员关心的是技术、工作流程和成本等方面的问题,消费者关心的是个人隐私问题,政策制定者关心的是法律、伦理以及可持续发展问题。此外,具有不同特征的同一类利益相关者对HIE的态度也存在群体性差异。美国的经验对于中国发展和应用HIE的重要启示是,由于不同利益相关者对HIE的态度存在差异,HIE的发展需要从医疗卫生行业的整体战略利益出发,发挥政府主导作用,加强顶层设计,最大化地满足不同利益相关者对于HIE的期望,从而保障HIE的可持续性发展。
Based on public literature review, it's found that Health Information Exchange(HIE) stakeholders(including medical institutions, physicians, patients and policy-makers) in the United States unanimously recognized the value of HIE in improving healthcare quality and reducing costs, but they have different considerations on the use and acceptance of HIE. Medical institutions and physicians care more about technology, working flow and cost issues, and consumers concern more about the personal privacy, whilst policy-makers put emphasis upon legal, ethnic and sustainable development issues. In addition, similar stakeholders with different characteristics also show different attitude toward HIE. The important implications from US experience for the HIE development and application in China are: developing HIE must bear in mind the overall strategic interest of the medical and health industry, give play to the leading role of government, strengthen top-level design and meet the expectations of different stakeholders on HIE to the maximum extent, so as to ensure the sustainable development of HIE.
