血清Cys C、β_2-MG联合尿α_1-MG、β_2-MG在亚临床肝肾综合征诊断中的意义 被引量:12

摘要 目的探讨肾功能正常肝硬化患者血清Cys C、血清β_2-MG;尿α_1-MG、尿β_2-MG的变化在亚临床肝肾综合征(hepatorenal syndrome,HRS)诊断中的意义。方法选取2011年8月-2013年8月符合标准的秦皇岛市第一医院住院肝硬化患者90例,根据不同表现及病因分组,采用生化仪检测血清β_2-MG、血清Cys C,尿α_1-MG、尿β_2-MG。结果肝硬化腹水组与肝硬化无腹水组、对照组比较,血清Cys C、血清β_2-MG、尿β_2-MG、尿α_1-MG明显升高(P<0.05);肝硬化无腹水组与对照组比较,血清Cys C、尿β_2-MG、尿α_1-MG轻度升高,差异无统计学意义(P>0.05);血清β_2-MG显著升高(P<0.05);不同病因肝硬化腹水患者之间血清Cys C、血清β_2-MG、尿β_2-MG、尿α_1-MG差异均无统计学意义(P>0.05)。结论血清Cys C、β_2-MG联合尿α_1-MG、β_2-MG可及早识别亚临床HRS,为HRS的早期预防和治疗提供依据。 Objective To investigate clinical significance of serum Cys C and β2-MG and urine α1-MG and β2-MG changes in patients with subclinical hepatorenal syndrome(HRS).Methods Ninety cirrhotic patients in the First Hospital of Qinhuangdao were chosen for the study from Aug.2011 to Aug.2013.According to clinical manifestations and causes grouping,serum Cys C,β2-MG,urine α1-MG,urine β2-MG were detected by biochemical analyzer,serum Cys C,serum β2-MG,urine α1-MG,urine β2-MG were compared in different groups of patients.Results Compared with cirrhosis without ascites group and the control group,serum Cys C,serum β2-MG,urine α1-MG,urine β2-MG were significantly increased in liver cirrhosis ascites group(P〈0.05).Compared with control group,serum Cys C,urine α1-MG,urine β2-MG were elevated in cirrhosis without ascites group,but there was no significant difference(P〉0.05).Serum β2-MG was significantly higher(P〈0.05);serum Cys C,serum β2-MG,urine α1-MG,urine β2-MG had no significant difference in different causes of liver cirrhosis ascites patients(P〉0.05).Conclusion Serum Cys C,serum β2-MG combined with urine α1-MG,urine β2-MG identify subclinical HRS to provide the basis for the early prevention and treatment of HRS.
