摘要
目的探讨可视膀胱镜联合输尿管镜治疗闭合性后尿道损伤的临床效果。方法 2011年8月至2015年7月收治的闭合性后尿道损伤患者78例,根据随机抽签原则分为治疗组与对照组各39例,治疗组采用可视膀胱镜联合输尿管镜手术治疗,对照组采用输尿管镜手术治疗。对比观察两组围手术指标、并发症、排尿症状评分、残余尿量和尿流率。结果治疗组的手术时间、术中出血量与术后住院时间明显少于对照组(P<0.05)。治疗组术后1个月的尿潴留、尿道出血、尿道狭窄、泌尿系感染等并发症发生情况明显少于对照组(P<0.05)。治疗组与对照组术后1个月的排尿症状评分分别为(5.33±1.87)分和(10.14±2.09)分,都明显低于术前的(23.14±2.19)分和(23.10±3.82)分,同时组间对比差异也有统计学意义(P<0.05)。两组术后1个月的尿流率明显高于术前(P<0.05),而残余尿量明显少于术前(P<0.05),同时治疗组术后1个月的残余尿量与尿流率和对照组对比差异也有统计学意义(P<0.05)。结论可视膀胱镜联合输尿管镜治疗闭合性后尿道损伤具有更好的微创性,能促进排尿功能的恢复,减少术后并发症的发生,有利于改善残余尿量与尿流率。
Objective To investigate the clinical effects of visual cystoscope combined ureteroscopy for treating closed urethral injury.Methods From August 2011 to July 2015, 78 closed urethral injury patients were selected, and according to the principle of random draw they were equally divided into the treatment group and the control group, the treatment group accepted visual cystoscope combined ureteroscopy treatment, the control group were given ureteroscopy surgery.Two groups of perioperative indicators, complications, urinary symptoms score, residual urine volume and urinary flow rate were observed.Results The operative time, blood loss and postoperative hospital stay in the treatment group were significantly less than those of the control group （P〈0.05）.The postoperative 1 month urinary retention, urethral bleeding, urethral stricture, urinary tract infections and other complications in the treatment group were significantly less than those of the control group （P〈0.05）.The postoperative 1 month voiding symptom scores in the treatment group and the control group were 5.33±1.87 points and 10.14±2.09 points that were significantly lower than those of the preoperative of 23.14±2.19 points and 23.10±3.82 points （P〈0.05） with statistically significant differences （P〈0.05）.The postoperative 1 month urinary flow rate were significantly higher than that of preoperative （P〈0.05）, and the residual urine volume was significantly less than that before surgery （P〈0.05）, while the postoperative 1 month urinary flow rate and the urine volume in the treatment group and the control group were different （P〈0.05）.Conclusion Visual cystoscopy combined with ureteroscopy treatment of closed urethral injury has better minimally invasive, it can promote the recovery of urinary function and reduce the incidence of postoperative complications, help to improve the residual urine volume and urinary flow rate.
