摘要
目的:对比分析导尿管、宫颈扩张球囊与米非司酮分别配伍米索前列醇在瘢痕子宫中期妊娠引产中促宫颈成熟的效果。方法:选取2015年1月-2016年12月于本院就诊的瘢痕子宫中期妊娠引产患者120例,采用随机数字表法分为A组、B组、C组,各40例。A组给予Foley’s导尿管,B组给予COOK宫颈扩张球囊,C组给予米非司酮,三组均配伍米索前列醇,对比三组促宫颈成熟的效果。结果:A组宫颈成熟总有效率与B组比较,差异无统计学意义(P>0.05);但两组宫颈成熟总有效率均显著高于C组(P<0.05)。A、B组在口服米索前列醇后规律宫缩开始时间、规律宫缩至胎盘娩出时间、引产出血量均显著优于C组(P<0.05)。A、B组引产失败率、软产道损伤率、发热率及胎膜胎盘残留率均显著低于C组(P<0.05)。结论:Foley’s导尿管与COOK宫颈扩张球囊在瘢痕子宫中期妊娠引产中促宫颈成熟的效果相似,能够有效缩短引产时间、减少引产并发症,引产失败率较低、安全有效,其中Foley’s导尿管与COOK宫颈扩张球囊相比较,价格便宜,容易取材,值得在基层医院中推广使用。
Objective：To compare and analyze the different effects of urethral catheter,dilatation balloon and Mifepristone in combination with Misoprostol on cervical ripening of induced abortion in the middle trimester of scar uterus.Method：A total of 120 cases with pregnancy of induced abortion from January 2015 to December 2016 in our hospital of uterine scar metaphase were divided into group A,group B and group C according to random number table method,40 cases in each group.Group A was given Foley＇s catheter,group B was received COOK balloon dilatation of the cervix,group C was received Mifepristone,they were combined with Misoprostol with each other.Effect of cervical ripening of three groups were compared.Result：There was no statistical significance in total effective rate of cervical ripening of group A and B（P〉0.05）;but total effective rate of cervical ripening of group A and B were significantly higher than that of group C（P〈0.05）.After taking Misoprostol,regular uterine contraction onset time,regular uterine contraction to placental delivery time and hemorrhage induced labor of group A and B were significantly better than those of group C（P〈0.05）.Failure rate of induced abortion,fever rate,soft birth canal injury rate and residual rate of fetal membrane and placenta of group A and B were significantly lower than those of group C（P〈0.05）.Conclusion：Cervical ripening effect of Foley＇s catheter and COOK balloon dilatation of the cervix in induction of labor of metaphase pregnancy of scar uterus are similar,both two methods can effectively shorten the induction time and decrease complications,their induction failure rate are low,the methods are both safe and effective,compared with COOK balloon dilatation of the cervix,the Foley＇s catheter is more cheaper,and it is more easier to obtain,so it is worthy to be popularized in primary hospitals.
