摘要
目的探讨系统性红斑狼疮（SLE）患者生活质量的影响因素。方法收集104例SLE患者的社会人口学资料，应用SLE病情活动指数（SLEDAI）评分和36条目健康调查简表（SF-36）分别评估其疾病活动情况和生活质量，应用抑郁症筛查量表9条目患者健康问卷（PHQ-9）、广泛性焦虑量表（GAD-7）、社会支持评定量表（SSRS）、匹兹堡睡眠质量量表（PSQI）及疲劳视觉模拟评分法（VAS）评估其心理状况并进行比较分析。结果SLE患者SF-36总分及其各维度得分均低于我国常模（P〈0．05）。SLEDAI、PHQ-9、GAD-7、PQSI、疲劳VAS评分与SF-36总分呈负相关（P〈0．01），SSRS评分与SF-36总分呈正相关（P〈0．01）。Logistic回归分析结果显示，疾病活动度、焦虑、社会支持、睡眠质量及疲劳为影响SLE患者生活质量的独立危险因素（R^2=0．860，P〈0．01）。结论SLE患者生活质量较正常人低，疾病活动度、焦虑、所获社会支持、睡眠质量及疲劳程度是其主要影响因素。
Objective To explore influencing factors on quality of life （QOL） in patients with systemic lupus erythematosus （SLE）. Methods Sociodemographic characteristics of 104 patients with SLE were recoMed. QOL and activity of SLE were assessed by 36-item Short Form Health Survey（SF-36） and SLE disease activity index （ SLEDAI ） respectively. Psychological status was evaluated by Patient Health Questionnaire-9 （ PHQ-9 ） , Generalized Anxiety Disorder-7 （ GAD-7 ）, Social Support Rating Scale （SSRS） , Pittsburgh Sleep Quality Index （PSQI） and fatigue Visual Analogue Scale （ VAS ） respectively. Results Scores of SF-36 and its all dimensionalities of patients with SLE were lower than those of general population（ P 〈 0. 05 ）. Scores of SLEDAI, PHQ-9, GAD-7, PQSI and fatigue VAS were negatively correlated with scores of SF-36（ P 〈0.01 ）. Scores of SSRS was positively correlated with scores of SF-36（P 〈0.01 ）. Logistic regression analysis showed disease activity, anxiety, social support, sleep quality and fatigue were the independent risk factors of QOL in patients with SLE （R^2 = 0. 860, P 〈 0. 01 ）. Conclusion QOL of patients with SLE is lower than that of general population. Disease activity, anxiety, social support, sleep quality and fatigue are the major influencing factors of QOL in patients with SLE.
