摘要
目的:探讨结节型神经纤维瘤的超声特征及鉴别要点。材料与方法:对14例经病理证实的结节型神经纤维瘤患者共67个瘤体的超声图像进行回顾性分析,灰阶超声观察瘤体的位置、分布特征、大小、形态、边界、内部回声及与相邻组织关系,CDFI观察瘤体内部及周边血流分布。结果:结节型神经纤维瘤可分布于全身各处,多见于躯干及肢体,多数位于皮下软组织,呈椭圆形低回声实质肿块、边界清晰、形态规则,瘤体两端与神经相连,呈"鼠尾征",瘤体内部或周边血流信号稀疏,呈点条状血流信号。结论:超声对结节型神经纤维瘤具有较高的诊断价值,超声发现瘤体两端与神经相连的"鼠尾征",仔细观察两者的连接方式,并结合临床,能与其他软组织肿物进行鉴别。
Objective:To investigate the ultrasonographic features and differential diagnosis of nodular neurofibromas.Materials and Methods:The ultrasonographic features of sixty-seven tumors in 14 cases of nodular neurofibromas confirmed by pathology were retrospectively analyzed.Sonograms were analyzed for the position,distribution,size,shape,margin,echog enicity and the relationship with peripheral tissue of nodular neurofibromas,CDFI was used to observe the distribution of the blood flow inside tumor and its outsid.Results:Nodular neurofibromas can be distributed throughout the body,common in trunk and limbs.A majority of tumors were located in the subcutaneous soft-tissue,showed oval solid hypoechoic mass and with clear boundary,regular shape.The masses and nerves were connected,that was " rat tail sign".Small amount of blood flow inside tumor and its outsid.Conclusion:Ultrasound is effective on diagnosis of nodular neurofibromas.Ultrasound found the two ends of the tumor and nerve connected with the " rat tail sign",and carefully observed the connection mode of the two and combine with the clinical can help to distinguished nodular neurofibromas from other soft tissue masses.
