摘要
目的混合痔急性发作时血液凝结成块状形成肿胀,血管内皮生长因子(VEGF)和巨噬细胞(CD68)等表达水平有关,本研究旨在探讨自拟凉血解毒洗剂治疗风热瘀阻型混合痔急性发作术后脓肿的疗效及对VEGF和CD68表达的影响,为临床治疗提供参考。方法选取2014年1月-2016年6月在我院接受治疗的110例风热瘀阻型混合痔急性发作术后脓肿患者为研究对象,随机分为对照组和观察组,每组55例,对照组采用高渗硫酸镁溶液坐浴和口服地奥司明片治疗,观察组在对照组的基础上给予自拟凉血解毒洗剂治疗,1周后观察2组患者的临床疗效,便血、坠胀感、黏膜、肛周脓肿中医证候积分,疼痛程度和水肿情况,免疫组化法检测VEGF和CD68表达情况。结果治疗后,观察组总有效率94.55%显著高于对照组74.55%(P<0.05)。观察组便血、坠胀感、黏膜、肛周脓肿中医证候积分显著低于对照组(P<0.05);观察组的疼痛评分和肛门水肿评分均显著低于对照组(P<0.05);观察组的VEGF和CD68水平明显低于对照组(P<0.05)。结论自拟凉血解毒洗剂治疗风热瘀阻型混合痔急性发作术后脓肿患者具有较好的临床疗效,可降低中医证候积分,改善疼痛和水肿,抑制VEGF和CD68表达。
Objective Mixed hemorrhoids acute postoperative abscess blood condense into clumps form swelling, the level of vascular endothelial growth factor （VEGF） and macrophages （CD68） expression were concerned to the disease. This study was to explore the influence of curative effect and expression of VEGF and CD68 ofLiangxue Jiedu lotion on treatment of mixed hemorrhoids acute postoperative abscess,to provide the reference for clinical treatment.Methods 110 patients with mixed hemorrhoids acute postoperative abscess were divided into control group and observation group according to random number table method from January 2014 to June 2016 in our hospital, and 55 cases were in each group. The control group was given high permeability magnesium sulfate hypertonic solution bath and oral diosmin piece while the observation group was givenLiangxue Jiedu lotion on the basis of the control group. After one week, the clinical efficacy, hematochezia, fall bilge feeling, mucous membrane, TCM syndrome integral of anus week abscess, degree of pain and edema were observed. The expression of VEGF and CD68 were detected by the immunohistochemical method.Results After treatment, the total effective rate 94.55% in observation group was higher than 74.55% of the control group （P〈0.05）. The scores of stool, fall bilge feeling, mucous membrane in observation group were lower than which of the control group （P〈0.05）. The pain score and the anus edema score in observation group were lower than which of the control group （P〈0.05）. The expression of VEGF and CD68 in observation group were lower than the control group （P〈0.05）.Conclusion The Liangxue Jiedu lotion on treatment of mixed hemorrhoids acute postoperative abscess has better clinical curative effect, which can reduce the TCM syndrome integral, improve pain and edema, inhibit VEGF and CD68 expression.
