摘要
目的:探究高脂血症胰腺炎伴肾衰竭患者给予血液灌流联合血液滤过的临床疗效。方法:选取2015年10月至2016年10月收治的患高脂血症胰腺炎伴肾衰竭患者100例作为研究对象,随机分为联合组和滤过组,每组50例。联合组给予血液灌流和连续静脉-静脉血液滤过联合治疗,滤过组给予连续静脉-静脉血液滤过治疗,观察两组患者临床疗效。结果:治疗前两组各项指标比较差异无统计学意义(P>0.05);治疗后联合组甘油三酯(triglyceride,TG)、血清总胆固醇(total cholesterol,TC)、白介素-6(interleukin-6,IL-6)、淀粉酶(amylase,AMS)、尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,Cr)、尿酸(uric acid,UA)均优于滤过组(t_1=3.725;t_2=3.213;t_3=2.694;t_4=3.050;t_5=2.475;t_6=3.088;t_7=1.956;P<0.05)。结论:血液灌流与连续静脉-静脉血液滤过联合应用可有效降低患者血液中的炎性反应产物浓度及血脂含量,改善患者肾功能指数,提高临床治疗效率,值得推广应用。
Objective：To explore the clinical effects of blood perfusion combined with hemofiltration on hyperlipidemia patients with acute pancreafifis and renal failure. Methods： 100 cases of hyperlipidemia patients with acute pancreatitis and renal failure from October 2015 to October 2016 were selected and randomly divided into combination group and filtration group, 50 cases in each group. The patients in combination group received blood peffusion and continuous intravenous - vein he- mofiltration treatment and those in filtration group were given continuous intravenous - vein hemofiltration treatment. The clinical efficacy was observed in both groups. Results： There was no statistieal significance of the indicators in the two groups before treatment （ P 〉 0.05 ） ; After treatment, triglyceride （TG）, serum total ehales- teral （ TC）, interleukin 6 （ IL - 6）, amylase （ AMS）, blood urea nitrogen （ BUN）, creatinine （ Cr ） and uric acid （ UA ） in combination group were better than those in filtration group （ t I =3.725; t 2 =3.213; t 3 =2.694; t 4 =3.050; t s --2.475; t 6 =3.088; t 7 = 1. 956; P 〈0.05）. Conclusion： Blood peffusion eom- bined with eontinuous intravenous - vein hemofiltration can effectively reduce the inflammatory reaetion product coneentration and blood lipid levels in the patient＇s blood, improve their index of renal funetion, and improve the efficiency of clinieal treatment, whieh is worthy of popularization and applieation.
