摘要
目的探讨^(99)mTc-甲氧基异丁基异腈(MIBI)双时相显像与SPECT/CT融合显像在原发性甲状旁腺功能亢进症(PHPT)病灶定位及定性的诊断价值。方法 20例PHPT患者均行^(99)mTc-MIBI双时像显像及SPECT/CT同机融合显像,显像结果与术后病理结果相比较,分析两种方法的灵敏度。结果 20例PHPT患者术后病理均确诊为PHPT(28个病灶),其中甲状旁腺腺瘤16例(21个病灶),甲状旁腺增生2例(5个病灶),异位甲状旁腺腺瘤1例(1个病灶),甲状旁腺癌术后淋巴结转移1例(1个病灶)。^(99)mTc-MIBI双时相显像与SPECT/CT同机融合显像敏感性分别为64.3%和85.7%,两种检查方法有显著性差异(P<0.05)。结论^(99)mTc-MIBI SPECT/CT同机融合显像是诊断PHPT的有效方法,其临床价值高于^(99)mTc-MIBI双时像显像。
Objective To evaluate the clinical value of ^99mTc-MIBI double-phase and SPECT/CT fusion imaging in diagnosis of primary hyperparathyroidism( PHPT). Methods This study included 20 patients with PHPT.^99mTc-MIBI double-phase and SPECT/CT fusion imaging were done in all cases. The imaging fingdings were compared to the pathological results. The sensitivity of two inspection methods was calculated and compared. Results 20 patients( 28 lesions) with PHPT were properly diagnosised with postoperative pathology. There were 16 cases( 21 lesions) of parathyroid adenomas,2 cases( 5 lesions) of parathyroid cysts,1 case( 1 lesion) of ectopicthyroid adenoma and 1 case( 1 lesion) of parathyroid carcinoma postoperative lymphatic metastasis. The sensitivity of^99mTc-MIBI double-phase was 64. 3%,and the sensitivity of SPECT/CT fusion imaging was 85. 7%,and the diagnostic sensitivity of SPECT/CT was higher than that of^99mTc-MIBI double-phase( P〈0. 05). Conclusion^99mTc-MIBI SPECT/CT fusion imaging is an effective method for diagnosis of PHPT,and with higher clinical application value in the diagnosis of patients with PHPT than^99mTc-MIBI double-phase.
