摘要
目的探讨4种肿瘤标志物联合检测在肺癌临床诊断的意义。方法应用电化学发光法测定肺癌组36例(病理检查已确诊)、良性组43例以及对照组36例(健康体检者)血清肿瘤标志物的表达水平,检测这3组人员血清肿瘤标志物CEA、CYFRA21-1、NSE、CA125,根据检测结果进行对比分析。结果肺癌组在血清癌胚抗原(CEA)、细胞角蛋白19片段21-1(CYFRA 21-1)、神经元特异性烯醇化酶(NSE)、和糖类抗原125(CA125)4种肿瘤标记物上显著高于良性组和对照组,差异均有统计学意义(P<0.05);良性组患者在CEA、CYFRA21-1、NSE、CA125 4种肿瘤标志物虽然略微高于对照组,与对照组比较分析,差异无统计学意义(P>0.05);4种血清肿瘤标志物联合检测与单项检测相比较,其敏感度、准确度更高,但在特异度上没有显著差异(P>0.05)。结论虽然肿瘤标志物单项检测对肺癌诊断中具有一定辅助诊断价值,但CEA、CYFRA21-1、NSE、CA125 4种肿瘤标记物联合检测可以进一步提高肺癌诊断的灵敏度和准确度,在临床上对肺癌的诊断及鉴别诊断都具有参考应用价值。
Objective Clinical significance of combined detection of fourtumor markers in the diagnosis of lung cancer.Methods Select 36 cases of patients with lung cancer（pathologically diagnosed）,43 cases of benign group and 36 cases in the control group（healthy people）,detected the expression level of tumor markers by electrochemiluminescence,the tumor markers of CEA,CYFRA21-1,NSE,CA125 were detected in 3groups,and the results were compared and analyzed.Results In the lung cancer group,carcinoembryonic antigen（CEA）and cytokeratin 19 fragment 21-1（CYFRA 21-1）,neuron specific enolase（NSE）and carbohydrate antigen 125（CA125）of4tumor markers were significantly higher than those of benign group and control group,the differences were statistically significant（P〈0.05）;benign groups of patients in CEA and CYFRA21-1,NSE CA125,although slightly higher than the control group,compared with the control group,the difference was not statistically significant（P〉0.05）;the sensitivity and accuracy of the combined detection of 4serum tumor markers were higher than that of single detection,but there was no significant difference in specificity（P〉0.05）.Conclusion Although the single tumor marker detection in the diagnosis of lung cancer has a certain diagnostic value,but combined detection of CEA,CYFRA21-1,NSE,CA125 can improve the sensitivity and accuracy in diagnosis of lung cancer,the clinical diagnosis and differential diagnosis of lung cancer has the reference value.
