摘要
目的观察阿替普酶联合神经节苷脂治疗溶栓时间窗外急性脑梗死患者的临床疗效。方法选取本院60例急性脑梗死患者,按照随机数字表法分为观察组(阿替普酶联合神经节苷脂)和对照组(神经节苷脂),每组30例。分别在治疗前和治疗后14d行CT灌注成像,对比大脑中动脉(MCA)的峰流速(Vp)、平均流速(Vm)的变化情况及进行NIHSS评分。结果观察组患者梗死中心CBV、CBF较治疗前增加,MTT较治疗前缩短,患者梗死周围区域CBV均较治疗前增加,梗死周围CBF较治疗前增加;治疗后梗死中心及梗死周围的rCBV、rCBF较治疗前均有明显增加;患者Vp、Vm显著升高,NIHSS评分显著降低。结论阿替普酶联合神经节苷脂治疗溶栓时间窗外急性脑梗死,能够改善脑梗死患者梗死中心及其周围区域组织灌注,尤其对梗死周围组织灌注的改善较为明显;可改善患者脑血流动力学,降低NIHSS评分,改善急性脑梗死患者神经功能缺损症状。
Objective To observe the clinical efficacy of alteplase combined with ganglioside in the treatment of acute cerebral infarction in patients with thrombolysis.Methods Sixty patients with acute cerebral infarction were randomly divided into observation group（alteplase combined with ganglioside）and control group（ganglioside）according to the random number table method.CT perfusion imaging was performed before and 14 days after treatment,and the peak flow velocity（Vp）,mean flow velocity（Vm）and the NIHSS score were compared between the middle cerebral artery（MCA）.Results The CBV and CBF of the infarct center in the observation group were significantly higher than those before treatment,and the MTT was shorter than that before treatment.CBV in the infarcted area was increased compared with that before treatment,and the CBF was increased compared with that before treatment.After treatment,the rCBV and rCBF Before treatment were significantly increased;patients Vp,Vm significantly increased,NIHSS score was significantly reduced.Conclusion Treatment of acute cerebral infarction with thalidomide in combination with ganglioside can improve the infusion of the infarct center and its surrounding area in patients with cerebral infarction,especially in the infusion of infarcted tissue.The improvement of cerebral hemodynamics Learning,reduce NIHSS score,improve the symptoms of neurological deficits in patients with acute cerebral infarction.
