摘要
目的分析1型糖尿病初发患儿外周血T细胞亚群(CD4^+、CD8^+、CD4^+/CD8^+)变化特点;探讨1型糖尿病初发患儿外周血T细胞亚群与胰岛功能的关系。方法选择2010-05~2013-12山西省儿童医院确诊的T1DM初发患儿43例,25例健康体检儿童为对照组。采用流式细胞技术测定血清CD4^+、CD8^+、CD4^+/CD8^+T细胞亚群,并对两组结果进行比较。对T1DM初发患儿T细胞亚群与糖化血红蛋白、C肽(C-P)水平进行相关性分析。结果 T1DM组患儿外周血CD4^+T细胞、CD4^+/CD8^+T细胞明显高于对照组(P<0.05),CD8^+T细胞明显低于对照组,差异有统计学意义(P<0.05)。T1DM患儿外周血CD4^+T细胞与Hb A1c呈正相关(r=0.261,P<0.05),与C-P呈负相关(r=-0.270,P<0.05);CD8^+T细胞与Hb A1c无明显相关性(r=-0.156,P>0.05),与C-P水平呈正相关(r=0.162,P<0.05);CD4^+/CD8^+T细胞与Hb A1c无明显相关性(r=0.200,P>0.05),与C-P水平呈负相关(r=-0.243,P<0.05)。结论 T1DM的发病过程存在T淋巴细胞亚群失衡,CD4^+T细胞异常激活、CD8^+T细胞缺陷和CD4^+/CD8^+T细胞比值增高。CD4^+及CD4^+/CD8^+T细胞与C肽呈负相关,CD8^+T细胞与C肽呈正相关。
Objective To investigate the changes of T lymphocyte subsets ( CD4^+, CD8^+, CD4^+/CD8^+ ) in peripheral blood of un- treated children with type 1 diabetes mellitus, and to explore its relationships with islet function. Methods Forty-three children with T1DM in Shanxi Children' s Hospital from May 2010 to December 2013 were selected as experimental group, and 25 healthy children were chosen as control group. The expression of CD4^+ , CD8^+ , and CD4 ^+/CD8^+ T cell subsets in two groups was measured by flow cytometry, and the results were compared. Correlation between T cell subsets and glycated hemoglobin, C peptide levels in T1DM chil- dren was analyzed. Results The levels of CD4^+ T cells and CD4^+/CD8^+ T cells in peripheral blood were significantly higher in T1DM group than in control group (t = 1. 413, 1. 424, P 〈 0.05), while the level of CD8^+ T cells was significantly lower(t = 1. 531, P 〈 0.05 ). The level of CD4^+ T cells in peripheral blood was positively correlated with HbA1 c ( r = 0.261, P 〈 O. 05 ), but negatively correlated with C-P ( r = - 0.270, P 〈 0.05 ). The level of CD8^+ T cells was not significantly correlated with HbA1 c ( r = - 0.156, P 〉 0.05 ), but positively correlated with C-P ( r = 0. 162, P 〈 0.05). The level of CD4^+/CD8^+ T cells was not significantly correlated with HbA1 c (r = 0. 200, P 〉 0.05), but negatively correlated with C-P(r = - 0. 243, P 〈 0.05 ). Conclusion T lymphocyte subses imbalance, such as abnormal activation of CIM + T cells, CD8^+ T cell deficiency and the increase of CD4^+/CD8^+ T cell, is involved in the process of the onset of T1DM. CD4^+ and CD4^+/CD8^+ T cells are negatively correlated with C-P, and CD8^+ T cells are posi- tively correlated with C-P.
