原发性高血压合并左心室肥厚与血浆同型半胱氨酸水平的相关性研究 被引量:11

摘要 目的探讨血浆同型半胱氨酸与原发性高血压合并左心室肥厚的相关性。方法选取南阳医学高等专科学校第一附属医院心内科自2014年3月~2015年1月收治的原发性高血压合并左心室肥厚患者42例,同期收治的原发性高血压患者45例作为对照组。所有研究对象均接受超声心动图的检查,并采集静脉血检测其血尿素氮(BUN)、血浆同型半胱氨酸(Hcy)、肌酐(Cre)的水平。比较两组研究对象的心脏超声参数左心室后壁厚度(LVPWT)、室间隔厚度(IVST)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF),以及上述指标与血浆中Hcy水平的相关性。结果与对照组相比,观察组血浆Hcy水平明显升高,结果具有统计学意义(P<0.001)。与对照组相比,观察组心脏超声参数LVPWT、IVST的水平均升高,结果具有统计学意义(P<0.001);LVEF的水平降低,结果具有统计学意义(P<0.05);观察组患者心脏超声参数LVPWT(r=0.396,P<0.05)、IVST(r=0.346,P<0.05)与血浆Hcy水平均呈显著正相关,LVEF(r=-0.336,P<0.05)与血浆Hcy水平均呈显著负相关。结论血浆Hcy水平与原发性高血压合并左心室肥厚的疾病严重程度具有相关性,对其诊断具有重要的临床意义。 ] Objective To investigate correlation between plasma homocysteine (Hcy) and left ventricular hypertrophy in essential hypertension. Methods 42 essential hypertension patients with left ventricular hypertrophy in The First Affiliated Hospital of Nanyang Medical College from March 2014 -2015 year in January were enrolled as observation group. 45 essential hypertension patients from the same period were selected as control group. All the subjects were examined by echocardiography, and the levels of Blood urea nitrogen (BUN), Hcy and Creatinine (Cre) were measured. Left ventricular posterior wall thickness (LVPWT), ventricular septal thickness (IVST), left ventricular end diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD), left ventricular ejection fraction (LVEF) were compared, and their correlation with Hcy level were also analyzed. Results Compared with the control group, the plasma Hcy level of the observation group was significantly increased, the results were statistically significant (P〈0.001). Compared with the control group, the levels of LVPWT and IVST in the observation group were increased, and the results were statistically significant (P〈0.001). Compared with the control group, the level of LVEF in the observation group decreased, the result was statistically significant (P〈0.05). There was a significant positive correlation between LVSTWT (r=0.396, P〈0.05) and IVST (r=0.346, P〈0.05) in the observationgroup. There was a significant negative correlation between LVEF (r=-0.336, P〈0.05) and plasma Hcy level in the observation group. Conclusion Plasma Hcy levels are associated with severity of hypertension in patients with essential hypertension and left ventricular hypertrophy.
