摘要
目的对比分析小切口非超声乳化囊外摘除术与超声乳化白内障吸除术对治疗老年白内障患者的临床疗效。方法方便选取该院2016年6—12月该院收治的131例老年性白内障患者,随机分为观察组65例使用小切口非超声乳化白内障摘除术,对照组66例使用超声乳化白内障吸除术,对比两组患者术前与术后围术期2、12、24、48 h眼压变化,术后视力恢复情况与不良反应情况。结果经治疗后,观察组术后视力恢复情况明显优于对照组,观察组术后2、12、24、48 h眼压分别为(16.81±4.33)、(25.11±14.71)、(19.91±3.98)、(13.99±3.47)mmHg明显低于对照组眼压(19.14±4.01)、(28.02±4.02)、(24.02±3.79)、(13.76±4.22)mmHg(P<0.05);观察组角膜水肿、虹膜损伤、瞳孔变化、前房积水、后囊破裂等不良反应发生率(15.38%)均低于对照组(34.85%)(P<0.05)。结论使用小切口非超声乳化囊外摘除术治疗老年性白内障患者较于超声乳化白内障吸除术效果更加显著,具有围术期眼压升高速度慢、术后视力恢复速度快且不良反应率低的优势,具有临床推广价值。
Objective To analyze and compare the clinical curative effect of different operations（small incision non-phacoemulsification cataract extraction and phacoemulsification for cataract surgery） in treatment of senile patients with cataract.Methods 131 cases of senile patients with cataract admitted and treated in our hospital from June to December2016 were conveniently selected and randomly divided into two groups,the observation group with 65 cases applied the（small incision non-phacoemulsification cataract extraction,while the control group with 66 cases applied the phacoemulsification for cataract surgery,and the eye pressure changes before operation and at 2,12,24,48 h after operation,postoperative vision recovery situation and adverse reactions situation were compared between the two groups.Results After treatment,the postoperative vision recovery situation in the observation group was obviously better than that in the control group,and the eye pressures at 2,12,24,48 h after operation in the observation group were obviously lower than those in the control group,[（16.81±4.33）,（25.11±14.71）,（19.91±3.98）,（13.99±3.47）mm Hg vs（19.14±4.01）mm Hg,（28.02±4.02）,（24.02±3.79）,（13.76±4.22）mm Hg]（P〈0.05）,and the incidence rate of adverse reactions such as corneal edema,iris injury,pupil changes,anterior chamber effusion and posterior capsule rupture in the observation group was lower than that in the control group（15.38% vs 34.85%）（P〈0.05）.Conclusion The effect of small incision non-phacoemulsification cataract extraction is more obvious than that of phacoemulsification for cataract surgery,which has the advantages of slow increase speed of eye pressure during the perioperative period,rapid postoperative vision recovery speed and low adverse reaction rate,and it is of clinical promotion value.phacoemulsification for cataract surgery;
