摘要
目的:通过Meta分析来评价聚乙二醇电解质溶液分次与单次口服对肠道清洁的效果及不良反应。方法:计算机检索Cochrane图书馆、Pub Med、CNKI、维普、万方数据库,检索年限为2000—2016年,全面收集聚乙二醇电解质溶液肠道准备的临床研究,采用Rev Man5.3软件进行Meta分析,以肠道清洁效果和恶心、呕吐、腹痛等不良反应作为观察指标。结果:共纳入6项随机对照临床研究,包括1532例患者。Meta分析结果显示,聚乙二醇电解质溶液分次与单次给药比较,肠道清洁效果较好,但两组间差异无统计学意义;分次组服药后恶心[OR=0.70,95%CI(0.61,0.81),P<0.00001]、呕吐[OR=0.20,95%CI(0.08,0.51),P=0.0006]、腹胀[OR=0.53,95%CI(0.42,0.67),P<0.00001]、腹痛[OR=0.49,95%CI(0.32,0.75),P=0.001]发生率均明显低于单次组,两组比较差异均有统计学意义;头痛、头晕[OR=0.59,95%CI(0.22,1.57),P<0.29]、睡眠障碍[OR=1.16,95%CI(0.90,1.49),P=0.25]两组间差异无统计学意义。结论:分次口服聚乙二醇电解质溶液肠道清洁效果好,服药后患者恶心、呕吐、腹胀、腹痛发生率低于单次给药组,因此提高了患者的依从性和耐受性,进而提高了肠道检查的准确性与安全性。
Objective: To conduct a meta-analysis to determine the effect and adverse reactions of PEG electrolyte solution bowel preparation by splitdosage versus non-split dosage. Methods: All clinical studies of PEG for bowel preparation were searched from Cochrane Library, Pub Med, VIP and WANFANG DATA between 2000 and 2016. Meta-analysis was performed using Rev Man 5.3 software. Observation index including bowel cleaning effectiveness and adverse reactions such as nausea, vomiting and stomachache were examined. Results: Six eligible studies involving 1532 patients were included. The results of meta-analyses showed that the effect of split dose group is superior to the single dose group. nausea[OR=0.70, 95% CI(0.61, 0.81), P〈0.00001]; vomit[OR=0.20, 95% CI(0.08, 0.51), P=0.0006]; abdominal distension [OR=0.53, 95% CI(0.42, 0.67), P〈0.00001] and stomachache [OR=0.49, 95% CI(0.32, 0.75), P=0.001] incidence rate of split dose group were inferior to single dose group; there was statistical significance; headache and dizziness [OR=0.59, 95% CI(0.22, 1.57), P〈0.29] and sleep disorders [OR=1.16, 95% CI(0.90, 1.49), P=0.25], there was no significant difference between2 groups. Conclusions: Split dose group provides a better colonic cleansing quality and the occurring rate of nausea, vomiting, abdominal distension, stomachache is lower than that of single dose group, so improving the patients' compliance and tolerance, and then improving the accuracy and security of colonoscopy.
