QRS时限对急性心肌梗死的诊断及预后的预测分析

摘要 目的探讨QRS时限用于急性心肌梗死的诊断和预后的临床预测价值。方法选取该院2014年1月至2016年10月收治的急性心肌梗死患者122例,随机分为对照组(QRS时限小于或等于100ms)和研究组(QRS时限大于100ms),每组61例。对两组患者的诊断和预后预测等相关情况进行对照研究。结果研究组患者N-末端脑肽钠前体水平,CK峰值,CK-MB峰值,心功能Killip分级为Ⅱ级、Ⅲ级、Ⅳ级的比例均明显高于对照组,左室射血分数(LVEF)、心功能Killip分级为I级的比例、血管不良事件总发生率均明显低于对照组,差异均有统计学意义(P〈0.05)。结论QRS时限用于触性心肌梗死的诊断和预后的预测的临床价值十分理想,能对患者心肌细胞电生理状况及心脏功能进行有效反映,且对患者急性肺水肿、恶性心律失常、心源性休克、心源性死亡等血管不良事件的发生具有临床预测价值。 Objective to investigate the clinical value of QRS duration in diagnosis and prognosis of acute myocardial infarction patients. Methods 122 cases of acute myocardial infarction from January 2014 to October 2016 were randomly divided into control group(100 ms or less QRS duration) and experimental group(〉 100 ms QRS duration), with 61 cases in each group. Case control study was conducted to research the diagnosis and prognosis prediction of patients in each group. Results the N-terminal brain pep- tide precursor sodium levels peak, peak CK,CK-MB of patients in experimental group were significantly higher than that of control group, and LVEF was significantly lower than that of control group, there were significant difference(t ~ 19.54,19.23,6.36,19. 23,P%0.05). Conclusion QRS duration has ideal clinical value in the diagnosis and prognosis of acute myocardial infarction pa- tients,it has effective response to heart function of patients and myocardial cell electrophysiology situation, and has clinical predic- tive value for the patients of acute pulmonary edema,malignant arrhythmia, cardiac shock, cardiac death and other blood vessels.
