摘要
目的探讨V、N形美容切口在腮腺区良性病变手术治疗中的应用价值。方法收集我院2014年1月至2016年10月手术治疗的腮腺区良性病变患者84例,V、N形美容切口组42例,常规S形切口组42例。除手术切口不同外,所有患者均解剖保护耳大神经、面神经、腮腺咬肌筋膜,切除肿物及周围0.5cm腮腺。根据缺损情况,选择胸锁乳突肌瓣、临近腮腺组织瓣、口腔修复膜等修复,跟踪观察手术的美容效果及术后并发症。结果采用两种切口治疗的腮腺良性病变患者,在手术操作时间、切口长度、术后暂时性面瘫、耳周区皮肤麻木、味觉出汗综合征、涎瘘等方面无明显统计学差异;V、N形切口瘢痕比较隐蔽,美容效果好,获得更高的满意度。结论与常规S形切口比较,V、N形切口并不会明显增加腮腺区良性病变手术操作的难度和并发症,具有切口隐蔽,瘢痕少,美容效果好,患者满意度高等优点。
Objective the research about the clinical value of V or N-shaped cosmetic incision in surgery of parotid gland benign lesion.Methods There were 84 cases of benign lesion in the parotid gland from January 2014 to October 2016 in our hospital, there were 42 cases of the V or N-shaped, and 42 of the S-shaped incision group. In addition to the surgical incision, all patients dissected and protected the Auriculotemporal nerve, the facial nerve, the parotid muscle fascia, the tumor were removed with 0.5 cm,observe the cosmetic efect of surgery and postoperative complications.Results Theoperation time, length of incision, postoperative temporary facial paralysis, ear area numbness, gustatory sweating syndrome and salivary fistula in Two group were no significant statistical diference.but V or N shape incision group have hidden scar, good cosmetic efect, a higher degree of satisfaction.Conclusion Compared with conventional s-shaped incision, V or N shape incision will not signifcantly increase the difculty and complication of benign parotid glandsurgery operation, and have hidden incision, less scarring, good cosmetic efect, high patient satisfaction.
