摘要
目的:观察消渴健脾胶囊对肥胖痰湿型2型糖尿病患者的临床疗效及对HOMA-IR、HOMA-β与炎性因子水平的影响。方法:将符合纳入标准的80例患者随机分为两组各40例。对照组给予基础治疗加盐酸二甲双胍片口服治疗,治疗组在对照组治疗基础上加服消渴健脾胶囊,治疗3个月后比较两组患者治疗前后中医证候疗效及血糖情况,计算HOMA-IR、HOMA-β,并检测炎性因子水平前后变化。结果:治疗组能显著改善患者的临床症状,中医证候疗效总有效率为85.0%,明显高于对照组的65.0%,差异有统计学意义(P<0.05);治疗组FBG、2h PG、HBA1C、FINS、HOMA-IR、hs-CRP、TNF-α及IL-6均较对照组显著降低,HOMA-β、IAI则显著升高,差异均有统计学意义(P<0.05)。结论:消渴健脾胶囊可提高2型糖尿病患者机体对胰岛素的敏感性、改善胰岛素抵抗、降低炎性因子水平,进而有助于改善血糖水平及临床症状,值得临床推广应用。
Objective ：To in,Jestigate the clinical effect of Xiaoke ]iaupi capsules in the treatment of type 2 diabetes with obesity and phlegm -dampness syndrome and its effect on homeostasis model assessment of insulin resistance（ HOMA -IR ） , homeostasis model assessmentβ - cell function（ HOMA -β） , and levels of inflammatory factors. Methods ： A total of 80 patients who met the inclusion criteria were randomly divided into two groups, with 40 palienls in each group. The pallents in the control group were given basic treatment combined with oral metfonnin hydrochloride tablets, and those in the treatment group were given Xiaoke Jianpi capsules in addition to the treatment for the control group. After three months of treatment, the two groups were compared in terms of the outcome of TCM syndrome and blood glucose before and after treatment ; HO- MA - IR and HOMA -β were calculated, and the changes in the levels of inflammatory factors after treatment were meas- ured. Results ：The treatment group had significant improvements in clinical symptoms and had a significantly higher overall response rate than the control group（85.0% vs 65.0% ,P 〈 0.05 ）. Compared with the control group, the treatment group had significant reductions in fasting blood glucose, 2 - hour postprandial glucose, HBA1 C, fasting insulin, HOMA - IR, high - sensitivity C - reactive protein, tumor necrosis factor - α, and interleukin - 6, as well as significant increases in HO- MA-β and insulin activation index（P 〈 0.05）. Conclusion： In patients with type 2 diabetes, Xiaoke Jianpi capsules can increase their sensitivity to insulin,improve insulin resistance, reduce the levels of inflammatory factors, and thus help to improve blood glucose level and clinical symptoms. Therefore, it holds promise for clinical application.
