摘要
目的 探讨行结肠下区动脉入路联合门静脉系统血管切除异体血管置换的根治性胰十二指肠切除术患者术后常见并发症的预防及护理对策.方法 分析首都医科大学附属北京朝阳医院肝胆胰脾外科2014年3月—2015年6月收治的行结肠下区动脉入路联合门静脉系统血管切除异体血管置换的根治性胰十二指肠切除术的13例患者术后并发症的发生情况,总结有效的护理干预措施.结果 13例患者术后未发生腹腔出血、消化道出血、门静脉血栓,胰漏3例,胃排空障碍1例,经积极治疗、护理后均康复出院.结论 通过对结肠下区动脉入路联合门静脉系统血管切除异体血管置换的根治性胰十二指肠切除术后患者严密的病情观察和有效的预防性护理,可积极预防、及时发现和治疗患者的并发症,挽救患者生命.
Objective To investigate the prevention and nursing of the complications of radical pancreaticoduodenectomy via artery approach under the colon area combined with portal vein and splenic vein resection and reconstruction.Methods A total of 13 cases of the radical pancreaticoduodenectomy via artery approach under the colon area combined with portal vein and splenic vein resection and reconstruction from March 2015 to June 2016 in Beijing Chao-Yang hospital were included in the study. Postoperative complications of patients were analyzed and effective nursing interventions were summarized.Results There was no patient with abdominal bleeding, gastrointestinal bleeding or portal vein thrombosis, 3 patients with pancreatic fistula and 1 patient suffering from delayed gastric emptying in total 13 cases. All of them were discharged after active treatment and nursing care.Conclusions Rigorous observation and effective preventive care on the patients of the radical pancreaticoduodenectomy via artery approach under the colon area combined with portal vein and splenic vein reconstruction could prevent, earlier observe and treat patients with complications to save their lives.
