可控式胸腔引流及冲洗装置治疗包裹性脓胸156例临床观察 被引量:5

摘要 目的:探讨用自制可控式胸腔引流及冲洗装置(专利号:201220433788.1)治疗包裹性脓胸的临床疗效。方法:我院2012-02~2016-08收治的包裹性脓胸患者156例,采用Seldinger法经皮穿刺留置导管行胸腔引流,并用生理盐水或甲硝唑冲洗脓腔,观察疗效。结果:156例患者均一次穿刺成功,术后2例出现引流不畅,经处理后再通;1例引流7d后导管滑脱,因复查示积液基本消失而拔管。引流时间3~35d,平均4.2d。第一次胸腔积液引流量≤800ml,每次引流时间>3h,引流1~8次不等。所有患者引流彻底,创伤小,症状明显好转。结论:采用经皮穿刺置管可控式胸腔引流及冲洗装置行胸腔引流是简单、方便、经济的脓胸治疗方法,能显著改善患者的生活质量,较大幅度减少患者行胸膜剥脱术的痛苦,适用于各级医院推广应用。 Objectives:To discuss the efficacy of self-made controllable thoracic drainage and flushing device in patients with encapsulated empyema.Methods:The data of 156 patients with encapsulated empyema from February 2012 to August 2016 in our hospital were collected.All patients were treated with thoracic drainage by Seldinger method of percutaneous puncture indwelling catheter,the normal saline and metronidazole were used to flush the abscess carity.After the treatment,the efficacy were observed.Results:All 156 cases were one time punctured successfully,2cases had poor drainage and patency drainage after treatment;1case catheter slippage occurred after drainage for 7 days,and the tube was removed because of the disappearance of the effusion.The drainage time was 3-35 d,with an average of 4.2d.The first drainage of pleural effusion ≤ 800 ml,each drainage time〉3h,drainage 1-8times.All patients had complete drainage,small trauma and obvious improvement.Conclusion:Thoracic drainage with controllable thoracic drainage and flushing device is a simple,convenient and economical method of treatment,which can significantly improve the quality of life of patients with empyema and significantly reduce the pain of patients with open pleural effusion,it’s suitable to adapt for the hospital.
