摘要
目的:探讨乌灵胶囊联合米氮平治疗更年期抑郁症的疗效以及对患者神经内分泌的影响。方法:采用随机双盲对照法将我院195例更年期抑郁患者随机分为对照组和治疗组,对照组给予米氮平治疗,治疗组给予米氮平+乌灵胶囊治疗。治疗8周,观察并比较两组患者的汉密尔顿抑郁量表(HAMD)评分和改良Kupperman指数评分(KMI)、世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分、不良反应症状量表(TESS)。治疗前后测定并比较两组患者血清单胺类神经递质5-羟色胺(5-TH)、去甲肾上腺素(NE)、雌二醇(E2)、促卵泡激素(FSH)、促黄体生成素(LH)水平。结果:对照组和治疗组的有效率分别为80.61%和92.78%(P<0.01)。治疗后2、4、6、8周与治疗前HADM评分比较2组均有明显降低,从治疗第2周末起,治疗组的HADM总分较对照组明显下降。治疗后两组间的KMI评分和WHOQOL-BREF评分比较均有显著性差异。治疗后两组患者的5-羟色胺(5-TH)和去甲肾上腺素(NE)较治疗前明显升高;治疗后两组患者的雌二醇(E2)、促卵泡激素(FSH)、促黄体生成素(LH)浓度较治疗前均有显著性差异。对照组的不良反应总数18例(18.37%),TESS值为(5.89±0.41)分;治疗组的不良反应总数8例(8.25%),TESS评分(3.14±0.28)分。治疗组的不良反应数及TESS明显低于对照组。结论:乌灵胶囊可提高米氮平治疗更年期抑郁症临床疗效,且不良反应少,依从性好,并能有效地调整患者的E2、FSH、LH水平,提高患者生存质量。
Objective： To explore the curative effects and safety of Wuling capsules combined with mirtazapine in climacteric depression. Meth- ods： 196 patients of climacteric depression were randomly divided into treatment and control groups. Mirtazapine was taken oral in control group（ n = 98） for 8 weeks. 98 patients of observation group took Wuling capsules combined with mirtazapine for 8 weeks. Clinical effective- ness and side effects were assessed with the hamilton depression scale（HAMD） and Kuppernlan scale and world health organization quality of life （WHOQOL-BREF） and treatment emergent symptom scale（TESS）. Results： The results showed that the effective rate of control group was 80.61%, and the effective rate of treatment group was 92.78% after 8 weeks interference, with significant difference between the groups. After two and four and six and eight weeks of the treatment, the score of HAMD decreased remarkably in both groups （ P 〈 0.01 ）. Compare with the control group ,HADM score of the treatment group were significantly decreased from the second treatment （P 〈0.05 or 0.01 ）. After eight weeks of the treatment, the scores of KMI and WHOQOL-BREF with significant difference between the groups. After eight weeks of the treatment,the serumlevels of 5-HT and NE increased, with significant difference between the groups（ P 〈 0.01 ）. After eight weeks of the treatment, the serumlevels of E2 and FSH and LH decreased obviously （P 〈 0.05 or 0.01 ）, with significant difference between the groups . The scores of TESS and the adverse rate of the control group were 5.89 0.41 and 18.37% ,and the scores of TESS and the adverse rate of the treatment group were 3.14 0.28 and 8.24%. There were significant changes existed in TESS and the adverse rate after treatment （ P 〈 0.05 or 0.01 ）. Conclusion： The results suggest Wuling capsules together with mirtazapine has good efficacy,less side effects and better compliance in the treatment of climacteric depression. This method deserve
