摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征（OSAHS）对急性脑梗死患者脑微出血（CMBS）的影响。方法连续选取经头颅MRI＋SWI证实的合并脑微出血的急性脑梗死患者60例，依睡眠多导图（PSG）检查结果分为对照组（非OSAHS组）和观察组（OSAHS组）。按呼吸暂停低通气指数（AHI）将观察组分为轻度OSAHS组、中重度OSAHS组。比较各组患者临床及影像学特征。结果观察组高血压、吸烟史明显高于对照组。观察组NIHSS评分高于对照组。影像学研究显示两组患者微出血病灶分布于幕上区、幕下区，观察组脑微出血病灶总数多于对照组（P〈0．05）。亚组分析显示中重度OSAHS组的脑微出血总数多于轻度OSAHS组（P〈0．05）。结论急性脑梗死合并OSAHS患者的脑微出血病灶更多，且与OSAHS的严重程度相关。OSAHS可能是急性脑梗死患者CMBs发生的危险因素。
Objective To investigate the effect of obstructive sleep apnea hypopnea syndrome （ OSAHS ） on cerebral microbleeds in patients with acute cerebral infarction. Methods Consecutive patients with acute cerebral infarction who had cerebral microbleeds shown by susceptibility weighted imaging （ SWI ） were enrolled to undergo polysomnography （ PSG ） .The patients were divided into two groups, the observation group （ cerebral microbleeds with acute cerebral infarction and OSAHS group ） and the control group （ cerebral microbleeds with acute cerebral infarction and without OSAHS group ） according to the results of PSG.The observation group was divided into two groups： mild OSAHS group and moderate- severe OSAHS group. The clinical and radiological features of cerebral microbleeds were compared among the three groups. Results A comparison of the risk factors showed that hypertension, a smoking history were more prevalent in patients with OSAHS than in those without OSAHS. The score of NIHSS in observation group was higher than the control group.Cerebral microbleeds were distributed in cortical subcortical area, the basal ganglia area and the infratentorial area.The number of cerebral microbleeds in the observation group was more than the control group.the number of cerebral microbleeds in moderate-severe OSAHS group was more than mild OSAHS group. Conclusions Patients with both cerebral infarction and OSAHS tend to have more extensive lesions of cerebral microbleeds. Cerebral microbleeds is associated with the severity of OSAHS.OSAHS can be a risk factor for cerebral microbleeds in patients with acute cerebral infarction.
