三维斑点追踪技术评价冠状动脉粥样硬化性心脏病患者左心室重构的价值 被引量:12

摘要 目的分析三维斑点追踪技术评价冠状动脉粥样硬化性心脏病(冠心病)患者左心室重构的价值。方法选取该院心血管内科2015年1月-2016年6月收治的冠心病患者50例作为观察组,以同期该院行体检的健康成年人50例为对照组,采集两组心脏容积数据,进行左心室体积几何学形态重建与对比观察以及心脏功能对比,发现心脏重构的特征。按照ASE的17节段划分法,分析左心室17节段心肌在长轴运动、径向运动及圆周运动上的应变变化。结果两组三维斑点追踪技术整体径向应变(GRS)、整体环向应变(GCS)、整体纵向应变(GLS)及整体面积应变(GAS)各整体心肌应变参数比较,观察组低于对照组,差异有统计学意义(P<0.05)。结论三维斑点追踪技术在评价早期冠心病左室功能收缩功能的变化方面具有较高的敏感性,可为临床评价冠心病患者左心室重构提供可靠依据。 Objective To evaluate the value of three-dimensional speckle tracking imaging in evaluating left ventricular remodeling in patients with coronary heart disease. Methods In this study, 50 patients with coronary heart disease treated in the Department of Cardiovascular Diseases of our hospital from January 2015 to June 2016 were selected as observation group, and 50 healthy adults having medical examination during the same period in our hospital as control group. Heart volume data acquisition, left ventricular volume geom- etry reconstruction in the two groups were collected, the cardiac function and cardiac remodeling in contrast were compared, and their feature were discovered. According to the 17 segment of ASE, the changes of 17 segment of the left ventricle in the long axis motion, the radial movement and the circumferential motion were analyzed. Results GRS, GCS, GLS, GAS of the overall myocardial strain parameters in the observa- tion group were significantly lower than those in the control group, and the difference was statistically signifi- cant (P〈 0.05). Conclusions Three dimensional speckle tracking imaging has high sensitivity in evaluating left ventricular systolic function in patients with early coronary artery disease, which can provide reliable evidence for clinical evaluation of left ventricular remodeling in patients with coronary heart disease.
