高频皮肤超声在鲜红斑痣患儿疗效评估的临床研究 被引量:3

摘要 目的探讨高频皮肤超声技术用于鲜红斑痣(port wine stains,PWS)患儿脉冲染料激光治疗疗效评估的可行性。方法选取在我院确诊为鲜红斑痣,头面部存在皮损病灶并接受脉冲染料激光治疗的患儿30例。所有患儿在接受脉冲染料激光治疗前、后均采用高频皮肤超声(丹麦高特司科技公司Derma Lab Combo皮肤生理检测仪)对皮损部位的皮肤厚度、密度值进行测量,并在治疗前,运用相同的方法测量皮损对侧正常皮肤的厚度及密度值。在患儿接受一个疗程(5次治疗)的治疗后,首先应用传统方法即治疗前、后单反数码照片评估皮损面积、颜色消退率,分为痊愈,显效、有效、无效四组。然后对四组治疗前、后皮损和对侧正常皮肤高频超声测量的厚度、密度进行分析。结果各组皮损与对侧正常皮肤厚度、密度比较无差异(p>0.05)。各组间治疗前皮损厚度、密度比较,痊愈组、显效组较有效组、无效组厚度薄,密度高。痊愈组、显效组分别与有效组,无效组皮损厚度、密度比较均有统计学意义(p<0.05)。PWS皮损治疗后,痊愈组、显效组、有效组皮损厚度、密度与治疗前比较,皮肤厚度均变薄,密度均增高。痊愈组,显效组,有效组各组皮损厚度、密度治疗前、后改变均有统计学意义(p<0.05)。无效组治疗后与治疗前比较皮肤厚度及密度改变均不明显,治疗前后改变无统计学意义(p>0.05)。结论高频超声技术能对正常皮肤、PWS病变皮肤进行皮肤厚度和密度精确测量,通过高频超声对PWS病变治疗前后皮肤厚度和密度精确测量可以作为鲜红斑痣患儿术前疗效预判、术后临床疗效评估的重要客观评价参考。 Objective: To investigate the feasibility of assessment using high frequency ultrasound for evaluating treatment effect of port-wine stains in children. Methods: 30 random patients diagnosed with port-wine stains, having craniofacial skin lesion, receiving pulsed dye laser therapy were selected. High frequency ultrasound (DermaLab Com- bo, Cortex, Denmark) was used to measure skin thickness and microvessel density before and after a course of pulsed dye laser treatment. Traditional method was used to evaluate the treatment effect. And according to the evaluating re- suits, patients were divided into 4 groups, including curative group, excellent group, effective group and invalid group. The results of high frequency ultrasound measurement of each group were analyzed. Results : There is no statis- tical significance in skin thickness and microvessel density of health skin among each group (p 〉0. 05 ). The skin thickness before treatment in curative group and excellent group are thiner than that in effective group and invalid group, and the microvessel density before treatment in curative group and excellent group are denser than that in effective group and invalid group. And there is statisf- ical significance between curative group, excellent group and effective group, invalid group respectively (p 〈0. 05). The skin thickness gets thiner and the microvessel density gets denser of curative group, excellent group and effective group after treatment. There is statisfical significance in the changes of skin thickness and microvessel density in curative group, excellent group and effective group (p 〈0. 05 ). And there is no statistical significance in the changes of skin thickness and microvessel density in invalid group (p〉 0. 05 ).
