甲状腺乳头状癌与代谢异常关系的研究 被引量:5

Relationship between papillary thyroid cancer and metabolic disorders

摘要 目的探讨甲状腺乳头状癌(PTC)患者糖类、脂类等能量物质的代谢情况。方法收集我院经病理确诊的PTC患者78例(PTC组),选择同期行甲状腺细针穿刺活组织检查(FNAB)的良性结节患者156例作为对照组。对所有患者行甲状腺超声、甲状腺功能和代谢相关指标检查并进行比较分析。结果PTC组患者的体重指数(BMI)、空腹血糖(FPG)、甘油三酯(TG)、促甲状腺激素(TSH)及尿碘均显著高于BTN组(P〈0.05)。多因素Logistic回归分析结果显示,年龄(OR=0.886,95%CI0.316—0.507)、BMI(OR=1.411,95%CI0.223-4.063)、FPG(OR=1.913,95%CI0.617~5.248)、尿碘(OR=1.549,95%CI0.750~5.310)是PTC的独立危险因素。结论BMI、FPG升高及碘过量可能是PTC的危险因素,对TG和TSH水平较高的甲状腺良性结节患者应高度关注,加强随访。 Objective To explore the association between papillary thyroid cancer( PTC ) and metabolism of energy substances, such as sugar, lipid and so on. Methods A retrospective analysis was performed on 78 patients with PTC diagnosed by pathology( PTC group). At the same time, 156 cases of benign nodules (BTN) diagnosed by thyroid needle biopsy (FNAB) were selected as control group. Thyroid ultrasound, thyroid function and metabolic indexes were compared and analyzed. Results Compared with BTN group, the patients in PTC group showed higher levels of body mass index ( BMI ) , fasting plasma glucose (FPG), serum triglyceride (TG), serum thyroid stimulating hormone ( TSH ) and urinary iodine( P 〈 0.05 ). In multivariate Logistic regression analysis, age ( OR = 0. 886,95% CI 0. 316-0.507 ), BMI( OR = 1.411,95% CI 0. 223-4. 063 ), FPG( OR = 1. 913,95% CI 0. 617-5. 248 ) and urinary iodine (OR = 1. 549,95% CI 0. 750-5. 310) were independently correlated with PTC. Conclusion The elevation of BMI, FPG and iodine overload may be risk factors for PTC. Meanwhile, benign thyroid nodules patients with high levels of TG and TSH should be highly concerned and follow-up carefully.
