螺旋CT扫描及后期处理对肺叶间裂的诊断分析

摘要 目的探讨螺旋CT扫描及后期处理显示肺叶间裂的诊断价值。方法采用64层螺旋CT对54例胸部CT扫描且未见异常患者和26例证实肺叶间裂附近病变的患者进行扫描及后期重建处理,分析不同层厚的斜裂、水平裂及叶间变异情况进行比较。结果1.25mm层厚斜裂显示情况,线状影占89.81%,显著优于2.50mm层厚的75.00%,差异有统计学意义(x2=8.51,P〈0.05);在2.50mm层厚线状影占75.00%,显著优于3.75mm层厚的42.60%,差异有统计学意义(x2=27.45,P〈0.05);水平裂在1.25mm层厚扫描影像线状影占11.11%,17.75%显示为乏血管区影,73.14%为带状影;对1.25mm层厚影像通过三维重建技术在矢状位、冠状位及横断面进行重组,结果显示右侧斜裂(X2=238.65,P〈0.05)、右侧水平裂(x2=328.79,P〈0.05)和左侧水平裂(x2=377.44,P〈0.05)比较,差异均有统计学意义。结论多层螺旋CT扫描后处理对于肺叶间裂的显示具有明显优势,特别是1.25mm层厚的重组影像,能够从不同方位进行分析,对于肺部疾病的诊断有很高的参考价值。 Objective To investigate the diagnostic value of spiral CT and post processing for pulmonary fissure. Methods 64-slice spiral CT scanning and image reconstruction were performed for 54 patients with no abnormalities in chest CT and 26 cases confirmed with lesions near pulmonary fissures. The oblique fissure, horizontal fissure, and interlobar variation shown under different slice thickness were analyzed and compared. Results As for the oblique fissure shown under 1.25 mm slice thickness, 89.81% images showed linear shadow, higher than the 75.00% of 2.50 mm slice thickness, with a statistical difference (X 2=8.51, P 〈 0.05); as for that shown under 2.50 mm slice thickness, 75.00% images showed linear shadow, higher than the 42.60% of 3.75 mm slice thickness, with a statistical difference (X 2=27.45, P 〈 0.05); as for the horizontal fissure shown under 1.25 mm slice thickness, 11.11% images showed linear shadow, 17.75% images showed vessel-poor shadow, and 73.14% images showed band-like shadow; the reconstruction of images obtained under 1.25 mm slice thickness was performed by three-dimensional reconstruction at sagittal, coronal and cross section, with the following results: oblique fissure of right lung (X 2=238.65, P 〈 0.05), horizontal fissure of right lung (X 2=328.79, P 〈 0.05), horizontal fissure of left lung (X 2=377.44, P 〈 0.05), with statistical differences. Conclusions Post-processing of multi-slice spiral CT scanning has advantages in showing pulmonary fissures, especially the reconstructed 1.25 mm image, contributing to multi-dimensional analysis. Therefore, it has high reference value in the diagnosis of nulmonary diseases.
