摘要
目的探讨妊娠34周以上未足月胎膜早破不同引产时间对妊娠结局的影响。方法本次共选择87例胎膜早破时孕周在34~36+6周的PPROM产妇作为研究对象,根据催产素使用距破膜的时间分为3组:A组23例、B组36例,C组28例,回顾性分析临床资料,比较3组孕产妇在住院过程中产前破膜时间、发生宫内感染、产褥感染、剖宫产率及新生儿窒息、新生儿呼吸窘迫综合征、新生儿感染情况。结果 A、B、C三组在宫内感染、产褥感染、剖宫产比例的比较均无统计学意义,而A、B两组与C组之间在产前破膜时间的比较,P<0.05,差异有统计学意义;A、B两组与C组在胎儿宫内窘迫、新生儿窒息、新生儿感染的比较,P<0.05,差异有统计学意义。结论妊娠34周以上的PPROM患者破膜24 h后引产组胎儿宫内窘迫、新生儿窒息、新生儿感染明显增加,严重影响围产儿结局。
Objective To investigate the effect of different induction time on pregnancy outcome in patients with preterm premature rupture of membranes （PROM） over 34 weeks. Methods The total of 87 cases of premature rupture of membranes in 34 to 36＋6 weeks PPROM pregnant women as the research object, according to the use of oxytocin from the rupture time were divided into 3 groups： A group of 23 cases, 36 cases of B group, C group of 28 patients, clinical data were retrospectively analyzed and compared 3 groups of pregnant women in the hospital before middle membrane rupture time, intrauterine infection, puerperal infection, cesarean section and neonatal asphyxia, neonatal respiratory distress syndrome and neonatal infection. Results A, B, C three groups in intrauterine infection, puerperal infection, cesarean section rate were not statistically significant, compared to A, B two group and C group in prenatal prom time, P 〈 0.05, the difference was statistically significant; two A, B group and C group in fetal distress, neonatal asphyxia, neonatal infection, P 〈 0.05, the difference was statistically significant. Conclusion After 24 h of rupture of membranes in patients with PPROM over 34 weeks gestation, the intrauterine fetal distress, neonatal asphyxia and neonatal infection in the induced labor group increased significantly, which seriously affected the perinatal outcome.
