摘要 目的:研究万古霉素硫酸钙颗粒填充治疗慢性骨髓炎的临床疗效.方法:对我院2010年7月~2016年4月收治的25例慢性骨髓炎患者进行彻底清除病灶结合万古霉素硫酸钙颗粒植入,并视患者具体情况予以皮瓣修复创面治疗,通过随访观察临床疗效.结果:20例切口Ⅰ期甲级愈合,5例切口处有渗液,经换药及应用重组人上皮生长因子后完全愈合,25例均取得良好效果,随访X光片见骨缺损处填充万古霉素硫酸钙颗粒逐渐吸收,新骨逐渐长入.所有患者术后功能均恢复良好.结论:万古霉素结合硫酸钙粉固化后形成颗粒较坚固,能替代植骨,作为一种骨移植替代物,颗粒能够在骨骼修复过程中逐渐被吸收,并被新生骨组织取代.由于这些颗粒是可生物降解和生物相容的,所以可用于被感染的部位,其具有填充和缓慢释放抗生素作用,且能够逐渐吸收,诱导骨长入,在解决骨缺损的同时,还可以维持局部抗生素浓度在较高水平,是治疗慢性骨髓炎的理想方法. Results: 20 cases of incision stage I healing class, 5 cases with incision have ooze liquid, the dressing and application of recombinant human epidermal growth factor after complete healing, 25 cases were obtained good results, follow-up X-ray films showed that the bone defects filled with vancomycin calcium sulfate particles gradually absorbed, new bone gradually. All patients recovered well after operation. Conclusion: Vancomycin combined with calcium sulfate to form a solid particles, can replace bone graft, as a bone graft substitute, particles can be absorbed in the process of bone repair, and was replaced by new bone tissue. Because these particles are biodegradable and biocompatible, so it can be used for the infected site has filled and slow release antibiotic effect and can be absorbed gradually, inducing bone ingrowth. In solving the bone defect at the same time, can also maintain local antibiotic concentration at a high level, is an ideal method for treatment of chronic osteomyelitis.
