摘要
目的 探讨造影技术及球囊临时阻断技术下行夹闭术与传统夹闭术治疗颈内动脉后交通动脉破裂动脉瘤的临床治疗效果.方法 选取颈内动脉后交通动脉破裂动脉瘤患者76例,随机分为研究组38例和对照组38例.研究组在造影技术及球囊临时阻断技术下行夹闭术,对照组行传统夹闭术.统计两组患者手术夹闭情况及并发症发生情况,采用格拉斯哥评价量表(GOS)评分评估两组预后情况.结果 研究组完全夹闭率(100%)略高于对照组(89.47%),但差异未见统计学意义(P〉0.05);研究组并发症发生率(7.89%)显著低于对照组(26.32%),差异有统计学意义(P〈0.05).研究组恢复良好率(78.95%)明显高于对照组(57.89%),差异有统计学意义(P〈0.05),重度残疾、植物生存、死亡发生率略低于对照组,但差异未见统计学意义(P〉0.05).结论 与传统夹闭术相比,造影技术及球囊临时阻断技术下行夹闭术治疗颈内动脉后交通动脉破裂动脉瘤夹闭效果好,并发症少,可明显改善患者预后.
Objective To compare the clinical effects of clipping under imaging techniques and balloon temporary blocking techniques and traditional clipping on posterior communicating ruptured aneurysms of internal carotid artery.Methods Seventy-six patients with posterior communicating ruptured aneurysms were randomily divided into study group (n=38) and control group (n=38).The study group was treated by clipping under imaging techniques and balloon temporary blocking techniques, while the control group was treated by traditional clipping.The condition of surgical clipping and complications were observed.The prognosis of the two groups were evaluated by the Glasgow Evaluation Scale (GOS).Results The total clipping rate in the study group (100%) was slightly higher than that in control group (89.47%, P〉0.05);The incidence rate of complications in the study group (7.89%) was significantly lower than that in the control group (26.32%, P〈0.05).The excellent and good rate of recovery in study group (78.95%) was significantly higher than that in control group (57.89%, P〈0.05).The incidences of severe disability, vegetative state and death were slightly lower than those in control group (P〉0.05).Conclusions Compared with traditional clipping, the effect of clipping under imaging techniques and balloon temporary blocking techniques on posterior communicating ruptured aneurysms of internal carotid artery is better, and the complications are fewer.It can significantly improve the prognosis of patients.
