摘要
目的通过实施PDCA循环理论,改进病案管理工作流程,提高病案管理质量。方法随机抽取2015年7月-2015年12月和2016年1月-2016年6月的出院归档病历2000份,按照病案整理顺序、病案借阅漏登、复印资料错误、编码错误、多份病案漏查5个方面进行检查,分析流程中存在的问题,运用PDCA循环法对主要问题进行干预并观察效果。结果实施PDCA半年后,病案管理工作问题错误率明显下降,病案管理质量达到了预期效果。其中编码错误率下降14%;病案整理顺序错误率下降12%;病案借阅漏登率下降7%;多份病案漏查率下降4%;复印资料错误率下降4%。结论 PDCA循环法能有效地规范病案管理的流程,更好的提高病案管理质量。在病案管理流程中,对于未解决的问题,在下一个PDCA循环中加以解决,从而构建病案管理质量的持续改进模式。
Objectives To promote the management work process of medical record and improve the management quality through the implementation of PDCA cycle theory. Methods 2000 cases of discharged file medical records during July to December 2015 and January to June 2016 were selected randomly, then conducted inspection from the five aspects included medical record sorting order, medical record lending leakage, photocopying, coding errors and the number of medical record missed in the two different periods, analyzed the problems existing in the process, and made intervention and observation on the main problems according to the method of PDCA cycle. Results After the implementation of PDCA after six months, the medical record management problem error rate significantly decreased, medical record management quality had reached the expected effect. In which the code error rate decreased by 14%; the medical record collating sequence error rate decreased by 12%; the medical record lending leakage rate decreased by 7%; the medical records check leakage rate decreased by 4% and the copying data error rate decreased by 4%. Conclusions The PDCA circulation method could effectively regulate the medical record management process, better improve the quality of medical record management. In medical record management process, we should solved in the next PDCA cycle if had some problems unsolved, so as to build the continuous improvement of quality of medical record management mode.
