摘要
目的:观察独活寄生汤治疗肝肾亏虚型腰椎间盘突出症的疗效。方法:将98例患者随机分为两组各49例,对照组给予腰椎牵引和手法推拿治疗,治疗组在对照组治疗基础上予独活寄生汤口服,比较两组治疗前后疼痛视觉模拟评分(VAS)和日本骨科协会评估治疗(JOA)评分,比较两组治疗前后血清丙二醛(MDA)、超氧化物歧化酶(SOD)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-1β(IL-1β)含量。结论:治疗后治疗组VAS低于对照组(P<0.01),JOA评分高于对照组(P<0.01),血清MDA、SOD、TNF-α、IL-6、IL-1β含量改善情况均优于对照组(P<0.01),治疗总有效率高于对照组(P<0.05),差异均有统计学意义。结果:独活寄生汤配合腰椎牵引和手法推拿治疗肝肾亏虚型腰椎间盘突出症可更有效缓解疼痛,改善腰椎功能,提高临床疗效,机制可能与减轻神经根无菌性炎性反应、改善氧化应激状态有关。
Objective： To observe the curative effect of Duhuojisheng decoction on lumbar intervertebral disc protrusion of liver- kidney deficiency. Methods： 98 patients were randomly divided into two groups, 49 cases in each group. The control group was treated with lumbar traction and massage manipulation, while the treatment group was treated with oral Duhuojisheng decoction on the basis of treatment of control group. Visual analogue scale （VAS） of pain and Japanese Orthopaedic Association （JOA） score were compared before and after treatment between two groups, and the contents of malondialdehyde （ MDA）, superoxide dismutase （ SOD）, tumor necrosis factor - α （ TNF - α ）, interleukin - 6 （ IL - 6） and interleukin - 1β （ IL - 1β） in serum were compared too. Results： After treatment, VAS of treatment group was lower than that of control group （P 〈 0. 01 ）, JOA score of treatment group was higher than that of control group （P 〈 0. 01 ）, the improvement of contents of MDA, SOD, TNF - α , IL - 6 and IL - 1β in serum of treatment group were better than that of control group （P 〈 0. 01 ）. The total effective rate of treatment group was higher than that of control group （P 〈 0. 05 ）, the difference was statistically significant. Conclusion ： In treatment of lumbar intervertebral disc protrusion of liver - kidney deficien- cy, Duhuojisheng decoction combined with lumbar traction and massage manipulation can more effectively relieve the pain, improve the function of lumbar spine, and enhance the clinical curative effect. Its mechanism may be related to reduction of radicular aseptic inflammatory reaction and improvement of oxidative stress state.
