摘要
目的探讨痴呆患者的精神行为症状(BPSD)与血清铜蓝蛋白水平的相互关系。方法选择符合ICD-10诊断标准的痴呆患者152例,根据痴呆程度不同分为轻度痴呆组、中度痴呆组、重度痴呆组,另设健康对照组。使用临床痴呆评定量表(CDR)评定患者痴呆的程度。分别进行铜蓝蛋白水平检测和简明精神症状评定量表(BPRS)评分,采用两因素方差分析精神行为症状与血清铜蓝蛋白水平和痴呆程度的相关性。结果中度痴呆组和重度痴呆组的血清铜蓝蛋白水平低于健康对照组和轻度痴呆组,差异有统计学差异(P<0.05);痴呆程度不同,BPRS得分不同;血清铜蓝蛋白水平不同,BPRS得分不同;痴呆程度与血清铜蓝蛋白水平具有交互作用,当患者处于中度痴呆且血清铜蓝蛋白水平接近于0.18 g/L时,患者BPRS得分最高。结论痴呆患者精神行为症状的严重程度与痴呆程度和血清铜蓝蛋白水平有关,临床可通过评估痴呆程度和检测血清铜蓝蛋白水平综合判断精神行为症状,制定更加合理的治疗方案。
Objective To explore the correlation between the behavioral and psychological symptoms of dementia（BPSD） and the level of serum ceruloplasmin（CER） of dementia patients. Methods 152 subjects were selected and divided into the mild, moderate and sever dementia group with a healthy group. Clinical dementia rating scale（CDR） was used to assess the severity of dementia. The level of serum CER was tested and brief psychiatric rating scale （BPRS） was evaluated. The means of two factorial anova analysis was used to analyze the correlation among BPSD, level of serum CER and the severity of dementia. Results The level of CER in patients of the mild and sever groups was significantly lower than those of the mild and healthy controls, with the differences statistically significant（P 〈 0.05 ）. The scores of BPRS were both relative with the severity of dementia and the level of serum CER and there were also interactions between severity of dementia and level of serum CER that patients with moderate dementia had the highest scores in BPRS when the level of serum CER was approximately to 0. 18 g/L. Conclusion There were interactions among severity of dementia, the level of serum CER and patients＇ behavioral and psychiatric symptoms. The BPSD could clinically evaluated through the severity of dementia and levels of serum CER in order to develop a more reasonable treatment plan.
