摘要
全髋关节置换术作为髋关节疾病终末期的有效治疗方法已经广泛应用于临床,可供骨科医师选择的手术入路及方法较多,直接前方入路(DAA)从自然肌肉解剖间隙进入不需离断髋部任何肌肉即可显露髋关节,对肌肉组织损伤小。随着近年来医学界对于新的微创技术的需求增加及快速康复外科的兴起,该技术也得到了广泛关注。与传统的术式相比,DAA具有创伤小、术后疼痛低、恢复快、住院时间短等优点,但也存在神经功能障碍、术中股骨骨折、学习曲线较长、操作复杂等问题。而对于骨科医师而言如何发挥DAA微创技术的优势,有效降低并发症的发生率则显得尤为重要。
Total hip arthroplasty as an effective method for the treatment of end-stage hip disease has been widely used in clinical,and there are plenty of surgical approaches and methods for the orthopedists, among which the direct anterior approach (DAA) can expose the hip without interruption of anatomical gap into the hip muscle causing less damage to the natural muscle. With the increasing demand for new minimally invasive techniques in the medical field in recent years and the rise of fast track surgery, the technology has received widespread attention. Compared with the traditional operation, DAA has the advantages of small trauma, low postoperative pain, rapid recovery and shorter hospital stay, but it also has the prob- lems of nerve dysfunction,intraoperative femur fracture,long learning curve and complicated operation. For the orthopedists how to develop the advantages of DAA minimally invasive technology, effectively reduce the incidence of complications is particularly important.
