摘要
目的探讨纤维支气管镜肺泡灌洗术治疗儿童大叶性肺炎的临床效果。方法选择2015年8月~2016年11月我院收治的80例大叶性肺炎患儿作为研究对象,根据随机数字表法分为观察组和对照组,各40例。对照组实施抗感染、止咳化痰、退热等常规药物治疗,观察组在对照组的基础上实施纤维支气管镜肺泡灌洗术治疗。比较两组的临床疗效、退热时间、咳嗽缓解时间、抗生素使用时间、肺部啰音消失时间、住院时间以及并发症发生情况。结果观察组的治疗总有效率为95.00%,显著高于对照组的77.50%,差异有统计学意义(P<0.05)。观察组的退热时间、咳嗽缓解时间、抗生素使用时间、肺部啰音消失时间、住院时间显著短于对照组,差异有统计学意义(P<0.05)。观察组的并发症发生率显著低于对照组,差异有统计学意义(P<0.05)。结论实施纤维支气管镜肺泡灌洗术治疗大叶性肺炎效果显著,能有效提高患儿的临床疗效,缩短治疗时间,降低并发症发生率,安全性较高,在临床应用中具有较高的推广价值。
Objective To explore the clinical effect of bronchoalveolar lavage（BAL） with bronchofibroscope（BFS） in the treatment of children with lobar pneumonia.Methods 80 children with lobar pneumonia in our hospital from August2015 to November 2016 were selected and divided into the observation group and the control group by random number table method,40 cases in each geoup.The control group was treated with anti-infection,relieving cough and reducing sputum,abatement of fever,the observation group was given BAL with BFS on the basis of the control group.The clinical effects,fever clearance time,cough alleviation time,antibiotics consumption time,extinction time of lung rale,hospital stays and occurrence of complications of the two groups were compared.Results The curative total effective rate of the observation group was 95.00%,which was higher than that of the control group（77.50%）,with significant difference（P〈0.05）.The fever clearance time,cough alleviation time,antibiotics consumption time,extinction time of lung rale and hospital stays of the observation group was shorter than that of the control group,with significant difference（P〈0.05）.The incidence rate of complications of the observation group was lower than that of the control group,with significant difference（P〈0.05）.Conclusion The effect of BAL with BFS in the treatment of children with lobar pneumonia is significant,it can effectively increase clinical efficacy,shorten treatment time,reduce the incidence of complications and have higher safety,which has higher promotional value in clinical application.
