摘要
目的分析急性脑梗死患者静脉溶栓治疗后继发脑出血转化(HT)的影响因素。方法回顾性分析我院69例急性脑梗死行静脉溶栓患者的临床资料,以溶栓后头颅CT或MRI检查作为判断标准分为无出血组及脑出血转化组,比较2组患者相关因素后,使用Logistic回归分析可能与静脉溶栓HT有关的因素。结果急性脑梗死患者69例,经静脉溶栓治疗后发生HT 12例,无HT 57例。2组年龄、既往卒中、早期急诊CT异常、溶栓前血糖、神经功能缺损评分(NIHSS)、尿酸、LDL、溶栓开始前舒张压、溶栓结束时收缩压及舒张压比较差异有统计学意义(P<0.05),进一步进行logistic回归分析发现NIHSS评分、溶栓前血糖、血尿酸、溶栓前舒张压与脑出血转化独立相关。结论溶栓前NIHSS评分、血糖、血尿酸和溶栓前舒张压可能是影响急性脑梗死静脉溶栓HT的相关因素。
Objective To analyse the revelant factors of intravenous thrombolysis in intracerebral hemorrhage transformation(HT) after acute cerebral infarction.Methods Retrospective analysis the clinical date of 69 cases patients with acute cerebral infarction treatment of intravenous thrombolysis.According to the skull CT and MRI examination,69 cases patients with acute cerebral infarction were divided into no bleeding group(57 cases) and intracerebral hemorrhage transformation group(12 cases).Compared the revelant factors of 2 groups.Analysis the relevant factors of intravenous thrombolysis by Logistic regression.Results In 69 cases patients with after acute cerebral infarction.12 cases patients with HT after intravenous thrombolysis,No HT in 57 cases.The age,past stroke,early emergency CT abnormalities,thrombolysis before blood sugar,neurological deficit score(NIHSS) Uric acid,LDL,thrombolysis before diastolic pressure,systolic blood pressure and diastolic blood pressure at the end of thrombolysis of 2 groups was no statistic significantly(P〈0.05).After logictic analysis,the independent correlation factors were NIHSS score,thrombolysis before blood sugar,blood uric acid and thrombolysis before diastolic blood pressure,which were independent correlation to intracerebral hemorrhage transformation.Conclusion The pre-thalamus NIHSS score,blood sugar,blood uric acid,thrombolysis before diastolic blood pressure may be the revelant factors to intravenous thrombolysis after acute cerebral infarction.
