摘要
目的研究目前SPECT/CT断层融合图像在诊断前列腺癌骨转移上的临床应用价值。方法选择2014年9月至2015年9月在笔者所在医院进行诊治前列腺癌患者180例。对患者进行99mTc-MDP静脉注射后利用SPECT行全身骨扫描加CT断层融合(SPECT/CT)检查。结果在接受检查的180例患者中,骨转移112例,其中胸骨及肋骨16例,颅骨转移16例,脊椎转移32例,四肢骨转移20例,骨盆转移28例。其中,SPECT全身骨扫描诊断灵敏度80.4%(90/112),特异性95.8%(68/90);SPECT/CT同机融合检查诊断灵敏度97.3%(109/112),特异性97.1%(68/71)。SPECT同机断层检查与SPECT全身骨扫描检查诊断灵敏度存在显著差异,结果有统计学意义[(χ2(0.05,1)=4.5,P=0.044<0.05]。结论 SPECT/CT断层融合显像对前列腺癌患者骨转移发现和诊断有着较高检出率和灵敏度,值得在临床广泛开展。
Objective To study clinical application value in prostate cancer with bone metastasis by using SPECT/CT. Method 180 patients with prostate cancer to be selected in Inner Mongolia People＇ s Hospital, and to have whole body bone scan with CT faultage image by injected with 99mTc - MDP. Results 112 patients was diagnosed bone metastasis in all 180 patients with prostate cancer. Among all these patients with bone metastasis, 16 patients have the bone metastasis in breastbone or ribs, 16 patients have bone metastasis in skull,32 patients have the bone metastasis in spine, 20 patients have the bone metastasis in limb bone, 28 patients have bone metastasis in pelvis. Among all the patients, the sensibility of SPECT whole body bone scan is 80.4 % （90/112）, the specificity is 95.8 % （68/90）; on the other side, the sensibility of SPECT/CT fusion imaging is 97.3% （109/112）, the specificity is 97.1% （68/71）. There was significant differences between SPECT/CT with fusion imaging and SPECT whole body bone scan in he sensibility, and there were statistical meanings in this result（χ2（0.05,1 ） =4.5, P=0.044〈0.05）.Conclusion SPECT/CT with fusion imaging had high de- tection rate and specificity to patients who had prostate cancer with bone metastasis in diagnosis, and it was worth to generalize.
