摘要
目的探讨促性腺激素释放激素激动剂(GnRH-α)联合反向添加疗法对于行腹腔镜保守手术的卵巢子宫内膜异位囊肿的临床可行性。方法将2014年1月~2015年12月在潍坊市益都中心医院行腹腔镜保守手术的卵巢子宫内膜异位囊肿患者180例,随机分成3组,对照组58例术后不予任何治疗,实验A组65例,术后给予GnRH-α治疗,实验B组57例,术后给予GnRH-α联合利维爱反向添加治疗,比较3组治疗后的激素变化、不良反应和复发情况。结果实验A组雌二醇(E2)、黄体生成素(LH)下降较实验B组明显(P<0.05)。实验组产生不同程度副反应,如潮热、盗汗、骨痛,但是实验B组的发生率较实验A组低(P<0.05)。实验组复发率分别为10.77%,8.77%,较对照组(24.14%)低(P<0.05),但实验组间比较,复发率无统计学意义(P>0.05)。结论卵巢子宫内膜异位囊肿术后GnRH-α联合反向添加疗法在临床上是可行且有效的。
Objective To analyze clinical efficacy of gonadotropin-releasing hormone agonist combined with add-back therapy for ovarian endometriosis cyst after laparoscopic surgery. Methods One hundred and eighty patients with ovarian endometriosis cyst in our hospital from January 2014 to December 2015 were randomly divided into 3 groups after laparoscopic conservative operation. The control group including 58 cases without any treatment, experimental group A including 65 cases were treated with GnRH-α, experimental group B including 57 cases were treated with GnRH-α combined with tibolone tablets. Hormone levels,adverse reactions and recurrence rates were compared between the three groups. Results After treatment, the levels of E2 （ estrogen ） , LH （ luteinizing hormone ） in experimental group A were statistically significantly lower than those in experimental group B （ P 〈 0.05 ）. Side effects such as hot flashes, ostealgia were shown in experimental groups, but the add back therapy can relieve these symptoms （ P 〈 0.05 ）. Recurrence rates in experimental groups were 10.77% - 8.77%, they were lower than that in control group 24. 14% （ P 〈 0.05 ） , the difference between experimental group A and experimental group B was not statistically significant （ P 〉 0.05 ）. Conclusion It is feasible and effective in the clinical application of GnRH- plus reverse addition therapy in the treatment of ovarian endometriosis.
