摘要
目的探讨PDCA循环在基层医院手卫生管理的应用及效果评价。方法选取斗门地区基层医院在职医护人员400例作为观察对象,调查分析其手卫生现况,应用PDCA循环在基层医院进行手卫生管理及评价,并根据调查结果制定干预措施并实施。比较干预前后医护人员手卫生问卷正确率、手卫生执行率、洗手规范率及手部细菌携带率。结果医院感染和手卫生状况调查结果显示,干预后医护人员对手卫生问卷正确率从干预前的47%提高到87%,手卫生执行率从干预前的37%提高到82%,洗手规范率从干预前的33%提高到91%,手部细菌携带率从干预前的26%下降到16%,比较差异均具有统计学意义(χ~2=144.731、168.067、285.569、12.055,P<0.05)。结论 PDCA循环在基层医院手卫生管理的应用,可提高本地区医务人员手卫生依从性,提高本地区医院感染管理水平,达到最终降低医院感染率的目标。
Objective To investigate application and evaluation of effect by PDCA circulation for hand hygiene management in primary hospital. Methods A total of 400 medical staff in primary hospital of Doumen district as study subjects received investigation and analysis of their current hand hygiene situation. PDCA circulation was applied in hand hygiene management and evaluation in primary hospital, followed by establishment and implement of intervention measures in accordance with investigation outcomes. Comparison was made on hand hygiene questionnaire accuracy in medical staff, hand hygiene implementation rate, hand washing standard rate and had bacterial carrying rate before and after intervention. Results Investigation outcomes of nosocomial infection and hand hygiene showed increased hand hygiene questionnaire accuracy in medical staff from 47% before intervention to 87% after intervention, hand hygiene implementation rate from 37% before intervention to 82% after intervention, hand washing standard rate from 33% before intervention to 91% after intervention, and had bacterial carrying rate from 26% before intervention to 16% after intervention. The differences all had statistical significance (χ^2=144.731, 168.067, 285.569, 12.055, P〈0.05). Conclusion Implement of PDCA circulation for hand hygiene management in primary hospital can enhance hand hygiene compliance in local medical staff and improve infection management degree in local hospital to achieve goal of reducing nosocomial infection rate.
