摘要
目的探讨通过MRI评估原发性肝癌化疗栓塞手术效果。方法选取2014年8月至2016年6月间新疆医科大学第二附属医院收治的68例原发性肝癌患者。所有患者经导管肝动脉化疗栓塞术前及术后1个月分别进行磁共振成像(MRI)检查,术后3个月行MRI增强扫描检查。根据实体瘤疗效评价标准将患者分为进展组与未进展组,比较两组患者的MRI图像特征。结果导管肝动脉化疗栓塞术(TACE)后,患者中有腹腔积液2例,肝门区胆管扩张3例,边缘结节样强化16例。疗效评价进展48例,纳入进展组,MRI结果显示弥散加权成像(DWI)呈现低信号,T1WI呈现高信号,T2WI表现为低或偏低信号,其中2例表现为边缘不光整;未进展组的信号特点与治疗前变化不明显,边缘不光整6例。两组患者的MRI图像特征比较,差异有统计学意义(P<0.05)。结论 TACE能够有效破坏肿瘤活性,可作为肿瘤根治性切除术的辅助和补充。
Objective To explore the efficacy of transcatheter arterial chemoembolization（ TACE）for primary liver cancer by using MRI. Methods Sixty-eight patients with primary liver cancer treated at The Second Affiliated Hospital of Xinjiang Medical University were selected from August 2014 to June 2016.All the patients underwent（ magnetic resonance imaging） MRI examination before and 1 month after TACE seperately. MRI enhancement scan was performed 3 months after operation. The patients were divided into two progression group and the non-progression group according to the solid tumors efficacy evaluation criteria（ RECIST1. 1）. MRI image characteristics were compared between the two groups. Results Among 68 patients treated with TACE,ascites was found in 2 patients,cholangiectasis in hepatic portal region was found in 3 patients,and marginal nodular enhancement was found in 16 patients. Forty-eight patients showed progression and were included in progression group. MRI displayed a low signal on diffusion weighted imaging（ DWI）,a high signal on T1 WI and a low or slightly low signal on T2 WI,among which irregular margin was noted in 2 patients,there was no obvious change in the signal characteristics in non-progression group before and after the treatment,and irregular margin was noted in 6 patients. There were significant differences in MRI image characteristics between the two groups（ P〈0. 05）. Conclusion TACE can effectively destroy the activity of tumor,and should be used as adjuvant and supplement for radical resection of tumor.
