营养师参与临床服务项目认证对患者饮食医嘱遵从性的影响 被引量:5

摘要 为分析营养师参与临床服务项目认证(CCPC)工作对患者饮食医嘱遵从性的影响,选取黄石市中心医院2015年11月至2016年7月纳入CCPC的急性心肌梗死(AMI)、卒中(Stroke)病例70例为观察组,未纳入CCPC的急性心肌梗死(AMI)、卒中(Stroke)病例60例为对照组。对照组常规由主管护士作饮食宣教,观察组按CCPC营养管理流程,由营养师实施营养健康教育,2周后对比分析2组患者饮食营养知识的掌握情况、治疗膳食订餐率、饮食医嘱的遵从率及对饮食营养健康教育的满意度。结果为:观察组患者对低盐低脂饮食营养知识的掌握情况显著好于对照组(P<0.05);观察组的治疗膳食订餐率高于对照组(x2=44.376,P<0.001);观察组患者低盐饮食遵从率、低脂饮食遵从率、低盐低脂饮食遵从率分别为60.0%,97.1%,54.3%,与对照组相比,两者差异有统计学意义(P<0.05);观察组患者对饮食营养健康教育的满意度评分(10.13±1.10)与对照组(4.35±1.44)相比差异有统计学意义(P<0.05)。营养师参与临床服务项目认证工作能有效提高患者饮食医嘱的遵从性。 Objective : To analyze the effect of dietitians in CCPC accreditation on the compliance of patients'dietary requirements in Huangshi Central Hospital. Method:Seventy cases of AMI (acute myocardial infarction) and Stroke patients in CCPC from November,2015 to July,2016 in our hospital were taken as observation group, and sixty cases who were not included in the CCPC of AMI and Stroke were treated as control group. The control group was routinely trained by the nurse, and the observation group carried out the education of nutrition and health by the dietitian according to the CCPC nutrition management process. After 2 weeks, comparative analysis was made between the two groups in the nutritional knowledge, ordering rate of therapy dietary,diet medication compliance rate and satisfaction of diet nutrition and health education. Results:The knowledge of low-salt and low -fat diet in the observation group was significantly better than that in the control group (P 〈 0.05 ). Ordering rate of therapy dietary in the observation group was higher than that of the control group (x2 = 44. 376, P 〈 0. 001 ). The compliance rates of low- salt diet, low- fat diet and low-salt and low -fat diet in the observation group were 60.0%, 97.1% and 54.3%, respectively, which were significantly lower than those of the control group ( P 〈0. 05 ). There was significant difference in the scores of satisfaction degree between the observation group (10.13±1. 10) and the control group (4.35±1.44) ( P 〈 0.05 ). Conclusion : The participation of dieticians in the JCI - CCPC certification process, can effectively improve the compliance of dietary advice in patients.
