摘要
目的探究老年乳腺癌患者的临床病理特点，为临床有效治疗、预后评估及护理实施提供参考与依据。方法选取2015年1至12月在本院就诊与治疗的老年乳腺癌患者（年龄大于60岁）52例，回顾其临床病理的诊断结果及其相关受体表达情况，分析不同年龄段患者病理状态之间的差异。结果研究结果表明，52例老年乳腺癌患者的病理诊断中，ER阳性患者共31例（59．62％），PR阳性患者共26例（50．00％），Her-2阳性患者共6例（11．54％）。随着年龄的增大，患者总体上出现肿瘤直径增大、肿瘤分级升高、非特殊类型浸润性导管癌发生率增加的情况，但差异无统计学意义（P〉0．05）。同时，患者的ER、PR的表达阳性率不断升高，Her-2表达阳性率不断降低，ER、PR的表达阳性率与Her-2呈显著负相关（r值分别为-0．681、-0．702，均P〈0．05）。结论可根据老年乳腺癌独特的临床病理特征，来提高其早期诊断率，选择适宜的术前诊断方法，积极采取个体化综合治疗，对于改善老年乳腺癌患者的生存质量及降低病死率有重大意义。
Objective To explore the clinical pathological features of elderly patients with breast cancer and to provide some references for clinical effective treatment, prognosis evaluation, and nursing implementation. Methods Fifty-two elderly patients with breast cancer who were treated at our hospital from January to December, 2015 were enrolled in this study. All the patients were reviewed and the clinical and pathological findings were reviewed. The expressions of relevant receptors and pathological results were reviewed. The differences of pathological states between patients with different ages were analyzed. Results The pathologic diagnosis of 52 elderly patients with breast cancer showed that 31 cases （59.62%） were positive in ER, 26 （50.00%） PR, and 6 （11.54%） Her-2. As the age increased, the patients＇ amaor diameter, tumor grade, and invasive ductal carcinoma incidence increased, but with no statistical differences （P 〉 0.05）. The positive rates of ER and PR negatively correlated with Her-2 （r = -0.681, -0.702, P 〈0.05）, and the positive rate of Her-2 expression significantly decreased （P 〈0.05）. Conclusions According to the unique clinical pathological features of elderly patients with breast cancer, increasing early diagnosis rate, selecting appropriate preoperative diagnosis method, and actively taking individualized comprehensive treatment have great significance in improving the quality of life of elderly patients with breast cancer and reducing mortality.
