摘要
目的比较高分辨率MRI与MR扩散加权成像(DWI)在直肠癌诊断中的应用价值。方法选择2012年6月至2016年8月本院收治的63例直肠癌患者为研究对象,所有对象均行MRI检查,比较单纯高分辨率MRI及高分辨率MRI联合DWI的T分期准确率及不同肿瘤分化程度患者的表观扩散系数(ADC)。结果高分辨MRI联合DWI的T分期准确率为88.89%,高于单纯高分辨MRI的73.02%(P<0.05)。粘液腺癌与腺癌不同分化程度(高、中、低)患者的ADC值之间有显著差异(P<0.05),分化程度越低,ADC值越低;粘液腺癌低分化、中分化及高分化患者的ADC值均高于腺癌患者(P<0.05)。结论与高分辨率MRI相比,高分辨MRI联合DWI总体T分期准确率更高,且ADC值与直肠癌病理分型及分化程度有关。
Objective To compare value of high-resolution MRI and MR diffusion-weighted imaging in the clinical diagnosis of rectal cancer. Methods 63 patients with rectal cancer were examined by MRI. The accuracy of T-staging and the apparent diffusion coefficient(ADC) of patients with different degrees of differentiation were compared between high-resolution MRI and high-resolution MRI combined with ADC. Results The accuracy rate of high resolution MRI combined with DWI was 88.89%, which was higher than that of high resolution MRI of 73.02%(P〈 0.05). The ADC values of mucinous adenocarcinoma and adenocarcinoma with different degrees of differentiation were significantly different(P〈 0.05). The lower the degree of differentiation, the lower the ADC value. Adenocarcinoma in poorly differentiated, moderately differentiated and well differentiated patients had higher ADC values than adenocarcinoma patients(P〈 0.05). Conclusion High-resolution MRI combined with DWI can improve the accuracy of T staging and help to provide reference for clinical treatment of patients. ADC value has a certain relationship with histological type and differentiation degree of rectal cancer, which is expected to be a noninvasive method for predicting the pathological types and differentiation of rectal cancer.
